How to Become a Modivcare Transportation Provider in 2027

Modivcare is the Medicaid ride broker that used to be called LogistiCare. Its operating company is ModivCare Solutions, LLC, and its headquarters is in Denver, Colorado. A state or health plan pays Modivcare to run its ride program, and Modivcare assigns each ride to a transportation company in its network. The company came out of Chapter 11 bankruptcy as a private company on December 29, 2025.

Where Modivcare runs Medicaid rides

As of September 2026, these programs are confirmed by the state or by Modivcare’s own program pages:

A few programs have limits worth knowing:

• Maine. Penquis CAP still arranged rides in Kennebec, Piscataquis, and Somerset counties and in Penobscot County outside Patten on the July 1, 2026 list. Maine DHHS awarded all eight transit regions to Modivcare in 2023. On August 4, 2026, the Maine Supreme Judicial Court upheld the awards for Regions 2, 3, 4, and 8 (2026 ME 79). • Oklahoma. Modivcare’s Oklahoma page also lists ride lines for the SoonerSelect plans from Aetna Better Health, Humana Healthy Horizons, and Centene Complete Health.
• Pennsylvania. Outside Philadelphia, rides run through county programs or state agreements with transit agencies, not a broker. The state’s June 25, 2026 study says its latest attempt to rebid the Philadelphia broker, RFA 06-24, was on hold during litigation.
• South Carolina. The state posted an intent to award its broker contract to MTM Health on July 3, 2025. On October 2, 2025, the Chief Procurement Officer granted part of Modivcare’s protest and found MTM’s proposal nonresponsive, and the state’s Procurement Review Panel upheld that finding. The state put the contract out to bid again on June 26, 2026. See South Carolina’s rebid.
• Utah. Modivcare’s Utah page says it schedules trips but does not pay for them, and it refers to the state’s fee schedule for transportation providers for items such as tolls. Ask Utah Medicaid how you enroll and bill before you take trips there.

Modivcare’s facility page lists state pages for more than 30 states, many of them for health plan clients. Check your state guide or NEMT brokers by state before you count on trips from a plan.

What changed for Modivcare in 2026

Modivcare won some programs and lost others this year. If you drive Modivcare trips, check each change against your service area:

Modivcare after Chapter 11

Modivcare filed for Chapter 11 on August 20, 2025, in the U.S. Bankruptcy Court for the Southern District of Texas. The court confirmed its plan on December 15, 2025, and the plan took effect on December 29, 2025. That day, ModivCare transferred substantially all of its assets to ModivCare Buyer, LLC under the plan, and said it would continue as a private company.

For drivers, the daily work did not change. MaineCare told members on February 6, 2026 that nothing had changed for people who ride with Modivcare. How to join Modivcare’s network

1. Check that Modivcare runs rides where you drive. Use the table above and your state guide. Modivcare’s join page also shows a map of the areas where it most needs providers.
2. Meet your state’s rules first. Get your business license, vehicle permits, and insurance in place. See the state rules below.
3. Fill out the Become a Transportation Provider form. It is on Modivcare’s Join Our Network page.
4. Wait for Modivcare to contact you. Use the Transportation Provider Support Form to ask about your application status.
5. Get your drivers and vehicles credentialed. Modivcare checks your documents, visits providers in the field to confirm vehicles meet contract standards, and offers training for NEMT work.
6. Start taking trips. Once approved, you use the Transportation Provider portal for billing, claims, and ride reservations.

What the form asks for

As of September 2026, Modivcare’s provider form asks for:

• Your company name, website, business name, address, and business start date.
• A contact person’s name, title, email, phone, and fax.
• Up to four owners.
• Your NPI and tax ID number.
• The services you offer: ambulatory (sedan, black car, or cab), wheelchair or ambulette, stretcher, basic and advanced life support ambulance, bus, and deviated fixed route.
• How many vehicles you own or lease, and your days and hours of operation.
• Your primary state and base county, plus a second state and the counties you serve there.
• The languages your team speaks and the dispatch software you use.
• Your general liability aggregate, auto liability combined single limit, and workers’ compensation amounts.

State rules that come first

• West Virginia. Under Chapter 524 of the state’s provider manual (effective June 1, 2024), non-ambulance transportation providers do not enroll with Medicaid. They must contract with the state’s broker and meet the chapter’s rules. You need a valid West Virginia business license and must stay current on workers’ compensation and employment security premiums and on state and local taxes. You also need liability coverage in the amount state law or the broker sets.
• Montana. Under the July 9, 2026 state notice, Modivcare takes over all NEMT provider enrollment and billing from the state’s claims contractor. Modivcare is reaching out to current providers with enrollment steps, and new providers enroll with Modivcare too.
• Every state. Under federal law, no NEMT provider or driver may be excluded from federal health programs, and every driver needs a valid license. South Carolina, for example, has applied these rules since December 15, 2021. Each provider also needs a process for any violation of a state drug law and a way to report each driver’s driving history to the state. Screen every driver against the OIG exclusion list.

What Modivcare requires from your company, drivers, and vehicles

Modivcare’s join page lists these requirements, as of September 2026:

Ask the network team for your program’s exact insurance limits before you buy or renew a policy. See NEMT insurance requirements. Prepare each van with the vehicle inspection checklist before a field visit, and train every driver on wheelchair securement.

How Modivcare sends you trips

Members, caregivers, and facilities book rides with Modivcare by phone, online, or through its facility portal. In Delaware, for example, Modivcare checks that the member is eligible and the trip is to a covered service, then arranges the ride with one of its contracted providers. In New Jersey, members call two days before an appointment and can book by phone up to a month ahead. In West Virginia, the manual asks members to call at least five business days ahead, and the broker tells you about each trip at least two calendar days before the appointment.

Modivcare’s state pages set out how trips work:

Modivcare also runs a marketplace where providers can take extra trips to fill gaps from cancellations, no-shows, or long waits. It offers its own dispatch software at no or reduced cost for Modivcare trips, and it connects with other dispatch systems. Log every pickup, drop-off, and signature on a trip log so each claim matches the trip.

How Modivcare pays

Your rate and payment terms are set in your agreement. West Virginia’s Chapter 524, for example, says the broker negotiates rates with each provider and pays them directly. It may use any payment arrangement both sides agree to, including a sub-capitation. The same chapter requires every broker agreement to cover payment administration and timely payment, so read those sections before you sign.

The Transportation Provider portal handles billing and claims, ride reservations, and booking driver safety courses. Payment timing depends on the program. On Virginia’s fee-for-service site, which Modivcare runs, the 2026 payment schedule looks like this:
