In Virginia, Modivcare arranges Medicaid rides for Humana Healthy Horizons and Sentara Community Plan members, and it still pays fee-for-service trips from before October 1, 2026, when MTM Health takes over that program. Stay credentialed with Modivcare to keep plan trips, and mail original trip logs for your last fee-for-service trips by the due dates on its 2026 payment schedule.

Modivcare, called LogistiCare until January 6, 2021, won DMAS’s fee-for-service broker contract for Virginia on January 16, 2018. That program moves to MTM Health with trips on October 1, 2026, and Modivcare keeps the rides it arranges for two Cardinal Care health plans: Humana Healthy Horizons in Virginia and Sentara Community Plan. What Modivcare keeps in Virginia and what moves to MTM
DMAS’s bulletin effective September 15, 2026 left every health plan’s broker in place and moved only fee-for-service:

The waiver row matters most for day support routes. Humana’s and Sentara’s 2026 member handbooks send rides to those three developmental disability waivers to the fee-for-service line at 866-386-8331, not to the plan. So from October 1, a van that carries plan members to a day support program needs an MTM agreement for those rides, while the same members’ doctor visits stay with Modivcare. The two health plans
Humana Healthy Horizons joined Cardinal Care on July 1, 2025, when Molina’s Virginia members moved to it automatically, according to DMAS’s provider FAQ of June 2025. Humana’s provider resource guide of January 2026 lists Modivcare as its required vendor for non-emergency medical transportation. Sentara Health Plans’ Medicaid provider manual, updated August 12, 2026, tells members to call its transportation vendor at 877-892-3986 at least five days before a covered service.
Modivcare’s Virginia member page, as of September 30, 2026, also lists ride lines for Sentara Health Medicare and D-SNP, Kaiser Permanente, and Wellpoint Medicare Plans. It still lists HealthSpring Medicare Advantage, but HealthSpring moved its Medicare Advantage rides to MTM Health on March 1, 2026. Ask Modivcare which of these programs your agreement covers before you count on their trips.
Finishing your fee-for-service trips before October 1, 2026
Bill every fee-for-service trip dated before October 1 to Modivcare, not MTM. The transportation provider FAQs on DMAS’s fee-for-service site set Modivcare’s process:
- Attach a billing invoice form to the original daily trip logs. The claims center requires both.
- Mail or ship the originals by USPS, FedEx, UPS, or another carrier. The claims center refuses faxed or emailed logs.
- Get them to the Claims Center at 798 Park Avenue NW, Norton, VA 24273, by 5 p.m. Eastern on the due date. MTM’s main Virginia office sits at the same street address, in Room 202A, so address the envelope to Modivcare’s Claims Center.
- Reconcile each payment against the Batch Detail Report. Denial reasons show on the Transportation Provider Detail Report, and pay for an authorized attendant shows in the Attendant column of the Batch Verification Detail Report.
- Resubmit a denied or missing trip with its trip log and the Resubmit Form.
The 2026 Accelerated Payment Schedule sets the dates. From the September 30, 2026 due date on, it reads:

Weeks not shown follow the same Wednesday and Thursday pattern. Ask the claims department how late it will accept fee-for-service trips before you plan on the final dates.
A few more rules from the same FAQs:
- Missing check. Wait at least 10 business days after the check run before you call the claims center, and allow 24 hours for a direct deposit to post.
- Trip number problems. The claims center cannot assign job numbers. Call the Exceptions Department at 866-809-4620, extension 2655.
- Mileage. Modivcare counts miles as the shortest street or road distance from pickup to destination. Raise a mileage dispute before you run the trip, because running it accepts Modivcare’s miles.
- Rates. The claims center pays the rates in your Transportation Agreement. Take rate questions to your Provider Relations Manager.
- Invalid driver or vehicle. That denial means a driver or vehicle fell out of compliance with your agreement. Call your regional manager and Modivcare’s compliance team right away.

Joining or staying in Modivcare’s Virginia network
You do not enroll with DMAS directly to give these rides. DMAS’s enrollment sheet, updated July 1, 2025, says each plan’s broker enrolls providers, trains drivers, inspects vehicles, negotiates rates, assigns trips, and pays, with requirements similar to, if not the same as, fee-for-service. Watch the phone numbers. The DMAS sheet of July 2025 listed Modivcare’s Virginia provider enrollment line as 866-810-8302 for Sentara and fee-for-service, and 866-603-8619 for Humana. As of September 2026, MTM lists 866-810-8302 as its own Virginia provider line. Humana and Sentara rules that shape your trips
Both plans book through Modivcare, but their rules differ:

Sentara rides over 50 miles, or out of state, need a signed medical necessity form from the member’s doctor and approval by Sentara, with three exceptions: Duke University Hospital, Children’s National Hospital, and Children’s Hospital of Philadelphia. Rides to those hospitals in Durham, Washington, and Philadelphia go ahead without that form.
Sentara’s provider sheet of October 2025 adds rules that decide which company gets a trip:
- Minors. Members 13 to 17 may ride alone with a Minor Consent Form on file. A child 12 or under with no parent or approved adult rides only with a provider that can supply a trained attendant, so a company with attendants can take trips others cannot.
- Escorts. Up to two escorts ride with a Sentara Community Plan member, and one with a Sentara Medicare member.
- Standing orders. Facilities send them through Modivcare’s TripCare site, or fax 1-866-907-1497, at least five days before the first ride. Modivcare’s Virginia facility page says every change goes through Modivcare, which then tells the assigned provider, so do not accept changes from the facility directly.
- Hospitals. A member rides from home to a hospital only when being admitted. Emergencies go to 911.

- List every unbilled fee-for-service trip from before October 1, 2026, and mail the originals by the next Wednesday due date.
- Sort your riders. Humana and Sentara medical rides stay with Modivcare. Fee-for-service members and waiver day support riders move to MTM.
- Get credentialed with MTM if you want to keep the fee-for-service and waiver work.
- Confirm your Modivcare agreement covers Humana and Sentara trips, and learn your provider relations region.
- Keep every driver and vehicle file current, since an out-of-compliance driver or vehicle turns a finished trip into a denial.

Does Modivcare still handle rides to developmental disability waiver services?
Not after September 30, 2026. Rides to Community Living, Family and Individual Support, and Building Independence waiver services go through the fee-for-service broker for every member, including Humana and Sentara members. From October 1, 2026 that broker is MTM Health, and day support facilities keep scheduling those standing rides.
How do I become a Modivcare provider in Virginia?
Get your DMV NEMT carrier certificate, bond, and insurance first, then send the Become a Transportation Provider form on Modivcare's website. You enroll with Modivcare, not with DMAS. Modivcare checks your drivers, vehicles, and insurance, and it asks for sexual abuse and molestation coverage on top of auto and general liability.
