Humana Medicaid transportation in Florida runs through Modivcare. Humana Healthy Horizons members book free rides at 866-779-0565, Monday to Friday, 8 a.m. to 5 p.m. Central, at least 24 hours before the visit, and long-term care members start with their Humana care coach. Humana Healthy Horizons in Florida is the Medicaid product of Humana Medical Plan, Inc., and Modivcare arranges all of its non-emergency rides. It is the largest Florida Medicaid plan that uses Modivcare, and the only one with members in all nine regions. Where Humana’s Florida riders live

AHCA’s enrollment report for August 31, 2026 counts 501,729 Humana members in Managed Medical Assistance (MMA), the main Medicaid program. Only Sunshine Health and Simply Healthcare had more. Humana also had 32,443 members in its long-term care (LTC) plan, again in every region. The regions follow section 409.966 of the Florida Statutes, and the Florida guide lists their counties.

Aetna Better Health and Community Care Plan also send their rides to Modivcare, in AHCA’s second quarter 2026 subcontractor report. The state’s NEMT waiver for July 2025 to June 2027 gives Modivcare the members outside a plan in Regions A, G, H, and I.

Regions B and C: Modivcare’s Medicaid work is Humana

In Regions B and C, MTM Health serves the members outside a plan, and neither Aetna nor Community Care Plan has members there. So from Duval and Alachua down to Pasco and Pinellas, the Medicaid trips Modivcare hands out are Humana trips. A company there that already works with MTM Health for UnitedHealthcare or fee-for-service riders adds Humana’s 120,902 members in those two regions with one more agreement. The Panhandle, where Humana leads

Humana is the largest Medicaid plan in three counties, all in Region A: Escambia, with 22,465 members or 49 percent of the county’s plan members, Santa Rosa, with 8,518 or 48 percent, and Walton, with 3,125 or 39 percent. In Escambia, Humana had more members than Sunshine and Simply combined, 22,465 against 21,737. It also has the most LTC members in Region A, 2,701 of 7,662, and Modivcare serves the region’s fee-for-service members too. Palm Beach and the Treasure Coast

Humana is the second-largest Medicaid plan in Palm Beach, with 48,236 members or 31 percent, behind Sunshine’s 66,275. It holds second place in Indian River (30 percent) and St. Lucie (29 percent) too, and in Okaloosa (35 percent) and Holmes (29 percent) in the Panhandle. Palm Beach also had 3,740 Humana LTC members in August 2026, second to Florida Community Care’s 3,806. Region G’s members outside a plan ride with Modivcare as well, so a Palm Beach County company with a Modivcare agreement can reach Humana, Community Care Plan, and fee-for-service riders from one broker.

The hours follow Central time. A rider in Broward or Miami-Dade who calls at 8:30 a.m. to book tomorrow’s visit is calling before the line opens, so tell your regular Humana riders to call after 9 a.m. Eastern.

The copay rule matters at the door. Florida’s NET policy sets a $1 copay per trip unless the member is exempt or the plan waives it. Humana’s 2026 provider handbook waives copayments for its MMA, HIV, serious mental illness, and LTC members 21 and older and tells providers not to charge them, and its member handbook, effective October 2026, lists no copays on any service for any member. Your driver collects nothing from a Humana rider.

How facilities book Humana riders

Clinics, dialysis centers, and nursing homes book Humana riders through Modivcare’s Facility Department, 1-866-252-1566 as of September 30, 2026. Members who ride three or more times a week to the same place go on a standing order. Modivcare’s facility rules, including how a facility asks for a preferred company, are on the Modivcare in Florida page. Long-term care members go through Humana first

Humana’s ride page tells LTC members to call their Humana care coach to schedule a ride, and its provider handbook says the member’s case manager approves rides to LTC services. Rides to non-medical appointments need prior authorization. In its Comprehensive plans, Humana’s contracts with assisted living facilities require the facility to coordinate transportation for its residents.

AHCA counted 19,886 of Humana’s 32,443 LTC members in its home and community-based services group in August 2026. The parent trips go wherever in Florida the child is placed, so check the miles on each one before you accept it. Humana also refunds meals on same-day medical trips more than 100 miles from home, up to $200 a day and $1,000 a year, for adults 21 and older with care manager approval. How to get Humana trips

Humana sends its members to Modivcare, so its trips reach transportation companies through Modivcare’s network.

  1. Enroll with Florida Medicaid. Florida’s plan contract, updated July 1, 2026, makes Humana confirm that every provider has a current AHCA agreement and finish onboarding within 60 days. The Florida guide walks through enrollment and the owners’ Level 2 screening.
  2. Apply to Modivcare. Send its Become a Transportation Provider form and list every Florida county you serve. The Modivcare page lists its driver, vehicle, and insurance checks, and Modivcare in Florida covers its other Florida plans.
  3. Get your county permit. Humana has members in every county, and several counties license transportation companies themselves. Miami-Dade certifies wheelchair and stretcher services, Broward licenses non-emergency medical transport, and Palm Beach permits non-medical wheelchair and stretcher companies under its vehicle for hire rules.
  4. Visit the facilities near you. Dialysis centers, nursing homes, and assisted living facilities book Humana riders and can name you as their preferred company.

What to do first

  1. Find your region in the table. In Regions B and C, count on Humana as your Modivcare Medicaid work.
  2. Start your AHCA enrollment, because Humana must confirm your AHCA agreement, and a plan can use a company with enrollment pending for only 60 days.
  3. Send Modivcare’s provider form listing every county you serve, and have your NPI, tax ID, and insurance papers ready for credentialing.
