Community Care Plan (CCP) is the trade name of South Florida Community Care Network, LLC, a provider service network owned by Broward County’s two public hospital districts: North Broward Hospital District (Broward Health) and South Broward Hospital District (Memorial Healthcare System). It covers Medicaid members, including a small Serious Mental Illness plan, in the 19 counties of Regions E to I, and Florida Healthy Kids members in eight counties from Indian River south to Monroe. Its office is in Sunrise. Florida lets each Medicaid plan hire its own ride broker, and the Florida guide covers state enrollment and rules.

Who books Community Care Plan rides today

Modivcare does. CCP’s Medicaid member handbook, dated July 1, 2026, lists ModivCare for non-emergency transportation, with reservations at 1-866-306-9358 and ride assistance at 1-866-306-9359. Its Medicaid provider manual of the same date lists the reservation line, and its Florida Healthy Kids quick reference guide gives 1-866-430-0570 for the children’s plan. AHCA’s plan subcontractor report through the second quarter of 2026 also lists ModivCare Solutions LLC for CCP.

Modivcare’s Florida facility page lists the CCP line and a Community Care Plan directions sheet, next to the standing order form facilities use for members who travel three or more times a week. CCP does not book rides with transportation companies itself, so its van trips reach companies through Modivcare and its Florida network.

CCP’s 2026 bid for a new ride vendor

On March 25, 2026, CCP issued a request for proposals for a company to run its whole ride benefit for Medicaid, the mental illness plan, and Florida Healthy Kids.

CCP expects a three-year agreement with up to two one-year renewals, and the bid plans at least 120 days for the winner to get ready. CCP may change the timeline by addendum. As of September 30, 2026, CCP’s July 2026 handbook and provider manual still name Modivcare.

This is a broker contract, not a van contract. The vendor runs a call center open around the clock for urgent trips and discharges, builds its own network of transportation companies, credentials them (CCP delegates credentialing), pays their claims, and sends encounter data. The bid asks for two years of audited financials, a Florida Medicaid reference, a $5 million umbrella policy, and $10 million of cyber coverage. For a small company, the bid decides whose network you need to be in. CCP’s bid notes that CCP is subject to Florida’s public records law, chapter 119, and that prices in the final contract are not confidential and may be disclosed.

Where CCP’s members live

On August 31, 2026, AHCA counted 69,566 CCP Medicaid members and another 117 in its Serious Mental Illness plan. By region, that was 6,995 in E, 10,851 in F, 12,817 in G, 34,011 in H, and 4,892 in I.

Nearly half of CCP’s members live in Broward, where it is the fourth-largest plan after Simply (60,046), Sunshine (58,283), and Humana (54,249). Humana also books through Modivcare, and in August 2026 the two plans together held about 88,000 of Broward’s 208,214 plan members. Outside Broward, CCP is a small plan: about 5 percent of plan members in Palm Beach and Lee, 8 percent in Collier, and 1 percent in Miami-Dade.

What CCP’s bid shows about its rides

CCP gave bidders its ride numbers for January to September 2025, covering its Medicaid, mental illness, and Healthy Kids members together.

At 4 percent, wheelchair work comes to about 590 trips a month across all five regions, and stretcher work to about 13. Ambulatory rides by sedan, van, or taxi are almost all of the work. About two trips in five run 11 miles or more.

Bidders priced the work two ways. One was a per-trip rate in five one-way mileage bands (under 6, 6 to 10, 11 to 25, 26 to 50, and over 50 miles) plus a per-trip administrative fee. The other was a full-risk rate per member per month, which is capitation. Your own rate still comes from your agreement with the vendor.

CCP’s bid lists the rules its vendor must follow, and says they become an exhibit to the vendor’s contract.

Hospital discharges, dialysis, wound care, chemotherapy, radiation, urgent care, and trips CCP itself requests need no notice. Members 16 and older, a parent, guardian, or authorized representative, and CCP case managers can book. Returns can be will-call when the member does not know how long the visit will take.

A few trips have their own rules:

  • Emergency rooms. No rides to an emergency room or between emergency rooms, but a ride home from one is covered.
  • Nursing homes. Home to a nursing home, a nursing home to a covered service and back, and one-way moves between nursing homes.
  • Pharmacy. Pharmacy runs count as non-medical trips.
  • The bus. Members who can walk, live within three quarters of a mile of a transit stop, and have an appointment within three quarters of a mile of one can be sent by bus.
  • Minors. Riders 13 to 16 can ride alone with a Consent for Minor Travel form signed by a parent. Children 12 and under need a parent, guardian, or approved adult, or a company that can send a trained attendant. See children in NEMT.
  • Service levels. Curb to curb, door to door, and hand to hand. The bid names riders with disabilities, nursing home residents, dialysis patients, and adult day care attendees as the ones who may need the last two.

Shared rides are allowed, including with riders from the vendor’s other clients. See multiloading.

CCP’s July 2026 handbook lists waived copayments on all services for Medicaid members 21 and older, so adult riders owe no trip copay. For Healthy Kids, CCP’s benefits page describes rides for the child and up to two companions to preventive medical and dental exams, with a $0 copay.

The standards CCP’s trips are measured against

The bid holds the vendor to these monthly numbers:

  • At least 90 percent of riders arrive on time for their appointment.
  • No more than 0.2 percent of requests end in a missed trip.
  • At least 85 percent of unscheduled trips, such as hospital discharges and urgent care, are filled within 3 hours of the request.
  • At least 90 percent of first legs are picked up within 15 minutes of the scheduled time, and 90 percent of returns within 30 minutes.
  • Complaints stay under 1 percent of rides each quarter.

Members with standing orders for dialysis, cancer treatment, or methadone may never miss an appointment because of a late pickup, a missed trip, or a transporter’s cancellation, and each case is counted from complaints made to AHCA. What CCP asks of vehicles, drivers, and claims

The bid’s service requirements set these rules for every vehicle and driver that carries a CCP rider:

On claims, the vendor must accept electronic claims from its network companies and pay 85 percent of clean claims for Medicaid rides within 7 calendar days, 95 percent within 10, and 98 percent within 20. For Healthy Kids rides the targets are 50 percent within 7 days, 70 percent within 10, and 90 percent within 15. No claim may be paid to a company or bank outside the United States.

How to get Community Care Plan trips

  1. Check your counties. CCP’s Medicaid members live in Regions E to I, nearly half in Broward. Its Healthy Kids members live in Broward, Miami-Dade, Monroe, Palm Beach, Martin, St. Lucie, Indian River, and Okeechobee.
  2. Get your local permits. Broward requires a county NEMT license, a permit for each vehicle, and a chauffeur registration for each driver; see Broward County. Palm Beach and Miami-Dade have their own permits.
  3. Enroll with Florida Medicaid. CCP’s bid bars any company from the vendor’s network unless it is eligible for Medicaid. The Florida guide walks through enrollment and owner screening.
  4. Apply to Modivcare’s Florida network through its Become a Transportation Provider form, and name Broward and the other CCP counties you serve. Humana, Aetna, and UnitedHealthcare also book Medicaid rides through Modivcare in Florida.
  5. Ask CCP about the vendor. Call CCP Provider Operations at 1-855-819-9506, Monday to Friday, 8 a.m. to 5 p.m., before you add vans for CCP work.
  6. Match your fleet to the mix. Ambulatory trips are 96 percent of CCP’s rides, and 34 percent run 11 to 25 miles.
