Modivcare in Oklahoma: SoonerRide, SoonerSelect Plans, and How to Join (2027)

Modivcare, called LogistiCare until January 6, 2021, arranges non-emergency rides for SoonerCare members in all 77 Oklahoma counties. It runs SoonerRide for members who stay in SoonerCare and books rides for all three SoonerSelect health plans, and its Oklahoma forms give an Oklahoma City office at 4149 Highline Blvd. The state license, insurance limits, and OHCA ride rules every Oklahoma company needs are in the Oklahoma guide, and Modivcare’s other states are in our Modivcare guide.

Who holds SoonerRide from October 1, 2026

OHCA pays one broker to run SoonerRide statewide, under a capitated method in its State Plan. It awarded the contract to LogistiCare on June 1, 2018. Since September 2025, OHCA has brought these steps toward a new contract to its board:

The June 2026 item asked for the 90 days “to support a smooth operational transition” to what it calls the newly awarded NEMT contract. As of September 30, 2026, OHCA’s SoonerRide member page names LogistiCare Solutions as its partner, and its one-pager revised April 16, 2026 names ModivCare Solutions. Before you plan around October trips, get two answers in writing from Modivcare’s provider relations director at 405-803-7532:

1. Does Modivcare hold the new SoonerRide term that starts October 1, 2026?
2. Does your transportation agreement carry over, with its rates and standing orders, or do you sign a new one?

OHCA’s procurement page routes questions about awarded contracts through its open records request form. The SoonerSelect plans contract for their own rides, so their trips stay with Modivcare unless a plan changes vendors.

Which Oklahoma riders Modivcare books

On April 1, 2024, SoonerSelect health plans took over care for more than half of SoonerCare members, while dual eligibles, aged, blind, and disabled members, people who need a nursing facility level of care, and several other groups stay in SoonerCare and ride through SoonerRide. The Oklahoma guide explains who belongs to each. For you, the difference is the booking line on the trip:

Modivcare’s Oklahoma facility page, updated June 24, 2026, also lists Medicare lines for Blue Cross and Blue Shield (HCSC) Medicare at 1-844-452-9381, Humana National Medicare at 1-866-588-5122, and HCSC group retirees at 1-866-824-1566. How much SoonerRide work there is

OHCA’s NEMT report for April to June 2026, dated July 27, 2026, counts SoonerRide only, not the three plans:

SoonerRide spending for the quarter came to $10,437,395. Among the three most common denial reasons, a member refusing the closest facility led with 1,646, followed by incomplete information or documentation with 1,235 and non-covered services with 1,101.

How to join Modivcare’s Oklahoma network

Oklahoma requires a Corporation Commission intrastate for-hire license to carry passengers, insured from $100,000 for vehicles with 6 or fewer seats, driver included, to $5 million for 16 or more. Stretcher vans also need a State Department of Health license, for a service area that is an emergency medical service region, an ambulance district, or a county of more than 500,000 people, and the Oklahoma guide has both processes. With those in hand:

1. Send Modivcare’s Become a Transportation Provider form with your base county and every county you serve. The Modivcare guide lists what the form asks and the driver checks that follow.
2. Ask for a dialysis-only contract if that is all you want. Under OAC 317:30-5-327.7, SoonerRide may contract with a company solely for dialysis trips. A nursing facility with its own vehicles can join as well, but it must meet the same vehicle, driver, safety, training, liability, and ADA standards, and it cannot limit service to its own residents except on dialysis runs.
3. Offer to cover areas short of providers. At OHCA’s January 17, 2024 board meeting, the CEO reported that Modivcare’s recruiters had signed a Joplin-based provider for northeast Oklahoma, a Dallas-based provider for the southeast, and a Texas provider bringing 10 new wheelchair vehicles that year, and that contracts were changed to let taxis give rides. 4. For a tribe, OHCA’s one-pager offers two ways to become a tribal transportation contractor, a contract or mileage reimbursement, and lists OHCA Tribal Government Relations at 405-522-7190. See tribal NEMT.
5. Keep every driver and vehicle file current. Since June 2026, OHCA has been reviewing its processes and guardrails for broker-contracted NEMT providers. The broker credentialing guide covers the file.

Rideshare limits on plan trips

From July 1, 2026, Amendment Ten to each SoonerSelect contract lets a plan use a rideshare company’s specialized medical program only for members who need no special assistance, and only if the plan chooses to. The same contracts keep members going to dialysis, surgical or sedation patients, members with high-risk pregnancies or in the third trimester, and members within six weeks after giving birth off the bus, so their trips need another mode, such as mileage reimbursement or a van like yours. The Oklahoma forms behind a trip

Modivcare’s Oklahoma facility page posts the forms that decide what you may carry and how a trip is logged. Facilities and doctors fill them in, so know which one each rider needs:

Stretcher service is only for members who cannot use a wheelchair and need no medical care during the ride, and Modivcare’s 2023 SoonerRide overview requires the certification on file first. Oklahoma trip rules that decide your pay

Modivcare’s Oklahoma facility page, updated June 24, 2026, its rider rules, and its 2023 SoonerRide overview set these:

Hospital discharge	A nurse, social worker, or discharge planner books it. Once it is arranged, you have up to 3 hours to pick up
Standing order change	Only Modivcare changes it, and you run from the daily manifest it sends, so send any change a facility gives you to Modivcare
Escorts	One escort for up to three riders. A nursing home sends one on every trip, and you need not carry the escort back without the member
Minors	Children under 15 ride with an adult, and riders 15 to 18 need a parent’s note on file with Modivcare
Extra stops	A pharmacy stop only when Modivcare adds it, with one small bag. No other stops and no packages

Service ends at the curb: drivers help riders in and out, fasten seat belts, and secure wheelchairs, but do not go into the home. SoonerRide does not cover bariatric wheelchair, bariatric stretcher, or advanced and basic life support levels. Humana Healthy Horizons lists wheelchair lift vehicles, including bariatric, on its Oklahoma ride page updated October 14, 2024, so a bariatric van may find plan trips that SoonerRide cannot send. A facility may ask for your company by name. Modivcare notes the request but assigns each trip by routing criteria that include provider compliance, and its 2023 overview warns that excessive grievances can cut the work you get or end it. How pay works for Oklahoma trips

Because OHCA pays the broker a capitated amount rather than a fee for each trip, Modivcare sets what it pays you in your transportation agreement. Read the rate schedule before you sign, and see negotiating broker rates. For scale, OHCA asked for up to $10,096,022 to run SoonerRide for the three months from July to September 2026.

Mileage reimbursement competes for the same riders. Modivcare’s 2023 overview encourages members with a car in the household to use it, three no-shows in 90 days can move a member to mileage reimbursement only, and nursing homes that drive their own residents claim $0.32 a mile on Modivcare’s form. Nursing homes also carry a cost you can take off their hands: under OAC 317:30-5-327.9, a home pays for any ride it does not book through SoonerRide and cannot bill the member for it, so it may hire you for those trips directly. Bill through Modivcare’s Transportation Provider portal. Get the rider’s signature on every trip log unless the standing order or an Unable to Sign Certification says the rider cannot sign.

What to do first

1. Call provider relations at 405-803-7532 and ask who holds SoonerRide from October 1, 2026 and whether your agreement carries over.
2. Get your Corporation Commission license and insure at the limit for your largest vehicle’s seats.
3. Apply on Modivcare’s form, list every county you will serve, and ask which ones are short of providers.
4. Pick your service levels. Dialysis-only is allowed. For stretcher work, confirm your area qualifies for a Health Department license before you buy a van.
5. Tell your dialysis centers and nursing homes which Modivcare forms each rider needs, so no trip waits on missing paperwork.
