Modivcare in Florida: Which Regions and Health Plans Still Send Trips (2027)

In Florida, Modivcare arranges Medicaid rides for members outside a health plan in Regions A, G, H, and I: the Panhandle, Palm Beach and the Treasure Coast, Broward, Miami-Dade, and Monroe. It also arranges rides for Humana members statewide and for Aetna Better Health and Community Care Plan members in their regions. To get these trips, enroll with Florida Medicaid, then send Modivcare's transportation provider form.

  • Modivcare runs fee-for-service Medicaid rides in Regions A, G, H, and I, and MTM Health runs them in Regions B to F.
  • Humana is the largest plan Modivcare serves in Florida, with 501,729 managed medical assistance members in all nine regions on August 31, 2026.

Modivcare, known as LogistiCare until January 6, 2021, came out of Chapter 11 as a privately owned company on December 29, 2025 (see Modivcare’s bankruptcy exit). In Florida it holds two kinds of work. The state pays it to arrange rides for Medicaid members who are not in a health plan in four regions, and three health plans pay it to arrange their members’ rides. Where Modivcare still sends trips in Florida

Two things decide whether a Florida Medicaid ride is Modivcare’s: the member’s plan, and, for members with no plan, the region they live in. The state’s NEMT waiver for July 1, 2025 to June 30, 2027 names Modivcare as the fee-for-service vendor for Regions A, G, H, and I. AHCA’s report of plan subcontractors, cumulative through the second quarter of 2026, lists it for Humana, Aetna Better Health of Florida, and Community Care Plan. It still lists UnitedHealthcare too, but that plan moved its rides to MTM Health on March 1, 2026. Laid over the plans’ regions in the August 31, 2026 enrollment report, that gives this map:

Under the 2023 to 2025 waiver, the same fee-for-service work sat in numbered Regions 1, 2, 9, 10, and 11, and older Modivcare forms still use those numbers. On AHCA’s map of the old regions, 1 and 2 became Region A, 9 became G, 10 became H, and 11 became I. In Regions B to F, fee-for-service members ride with MTM Health.

Humana holds about seven in ten of these members. Humana’s Florida page asks members to call Modivcare at least 24 hours before a visit, and long-term care members schedule through their Humana care coach. Sunshine told members that their Modivcare standing orders would carry over to Alivi. UnitedHealthcare announced its move in its March 2026 provider overview, and HealthSpring made MTM its only ride provider for Medicare Advantage members. Modivcare’s own Florida pages still listed both plans on September 30, 2026. The facility page shows the old UnitedHealthcare Medicaid lines (1-866-372-9891 for MMA and 1-877-931-4751 for long-term care), but UnitedHealthcare’s 2026 provider manual gives MTM’s line, 1-888-777-6528, so give that number to UnitedHealthcare riders and facilities. The member page lists HealthSpring Medicare Advantage at 1-866-214-5126 for North, Central, and South Florida. HealthSpring kept that number, and MTM now answers it.

A fourth change may come. Community Care Plan put its ride contract out to bid on March 25, 2026, with a notice of intent to award set for May 22, 2026, and asked bidders to confirm they could start within a 120-day implementation period. Its member handbook dated July 1, 2026 still names Modivcare. Confirm CCP’s vendor before you add vans for its Broward and Palm Beach members.

The facility page also lists Medicare and retiree programs: Humana Medicare (1-866-779-0567), UnitedHealthcare Medicare National (1-866-418-9812), Medica HealthCare and Preferred Plans (1-888-774-7772), Blue Cross Blue Shield (1-855-875-5194), and HCSC and Humana group retiree plans. Ask which of them your agreement covers.

Fee-for-service riders in Regions A, G, H, and I

Under the waiver, fee-for-service riders include people on Medically Needy, presumptively eligible pregnant women, women in the Breast and Cervical Cancer program, and members who may stay out of a plan, such as children at a prescribed pediatric extended care (PPEC) center, iBudget waiver members, developmental disability group home residents, and people with other coverage besides Medicare. Members with Medicare, and members of the long-term care waiver, are left out.

AHCA pays Modivcare a set amount each month for every eligible member, a capitation arrangement, with PPEC children in their own rate group. Modivcare then pays you at the rate in your transportation agreement.

Modivcare’s Florida member handbook for AHCA members, as posted in September 2026, sets the rules your drivers work to:

  • Booking. Routine rides need 3 business days of notice at 1-866-726-1457, weekdays 8 a.m. to 5 p.m. Urgent rides are taken any time, and same-day rides may take up to 3 hours.
  • Cancellations. Members cancel at least 24 hours ahead on Ride Assist, 1-866-726-1458, or the trip may count as a no-show.
  • Late rides. A ride is late once it is 15 minutes past the scheduled pickup, and members report it to Ride Assist as a Provider Late complaint.
  • Will-call returns. The driver may take up to 60 minutes to arrive after the call. See will-call trips.
  • Assistance. The handbook lists curb-to-curb help, plus door-to-door and hand-to-hand service when medical necessity is requested.
  • Riders. One helper may ride. Children 16 and younger need an escort 18 or older, or a consent form on file, and members bring their own child safety seat.
  • Vehicles. Rides go by car, van, or taxi, volunteer driver, wheelchair van, stretcher van for a member who must lie down but needs no care, or non-emergency ambulance when care is needed on the way.

The PPEC trips behind a corrective action plan

The waiver reports that Modivcare missed two timeliness standards in August, September, and October 2024: arrival within 15 minutes of the appointment time, and arrival at or before it. AHCA put it on a corrective action plan in December 2024 that ran to July 31, 2025. Most of the late trips were to PPEC centers, which usually set the appointment time as the time the transportation escort goes out to pick up the child, not the time the child must arrive, on recurring trips booked for the same time each day. Modivcare builds its timeliness report from claims data, and it said some providers had switched digital platforms and were retraining drivers.

If you run PPEC standing orders, ask each center what time the child must arrive, and have drivers record every pickup and drop-off as it happens.

Where Modivcare’s Florida vehicles are

The waiver’s 2025 capacity table counts 2,040 vehicles in Modivcare’s Florida network: 1,106 ambulatory, 801 wheelchair, and 133 stretcher. Of those, 1,358 were in its four fee-for-service regions, and the other 682 were in counties outside those regions. The counties with the most:

The same table lists no vehicles at all in Franklin, Jefferson, or Taylor counties in Region A, or in Indian River, Martin, or Okeechobee counties in Region G. Monroe County had 11, none of them wheelchair vans. The waiver requires each vendor to use companies outside its network when its own cannot serve a member, and says the two vendors have covered for each other outside their regions until the assigned vendor found a provider. Modivcare’s provider page also shows a map of the areas where it most needs providers.

Miami-Dade, Broward, and Palm Beach counties add a local license. Miami-Dade County requires a Non-Emergency Certificate for wheelchair and stretcher companies, Broward County licenses non-emergency medical transport services, and Palm Beach County permits wheelchair and stretcher companies as vehicles for hire. Get that permit before you apply.

How to join Modivcare’s Florida network

  1. Enroll with Florida Medicaid through AHCA as a non-emergency transportation provider, and book Level 2 screenings for your owners. The steps are in the Florida guide.
  2. Get your county permit if you run in Miami-Dade, Broward, or Palm Beach.
  3. Send the Become a Transportation Provider form on Modivcare’s Join Our Network page, with the services you offer, your vehicle count, and your primary service state.
  4. Pass credentialing. Each driver needs a background check, a drug screen, a clean motor vehicle record, first aid and CPR, defensive driving, and a PASS wheelchair certificate. Each vehicle needs current registration, any required inspection, and insurance.
  5. Carry the insurance Modivcare lists: general liability, sexual abuse and molestation coverage, auto liability, and workers’ compensation where Florida requires it. Ask for the Florida limits in writing.
  6. Confirm your programs. Ask which regions, plans, and Medicare programs your agreement covers.

Aetna adds a billing rule. Its June 17, 2025 bulletin says that when Aetna is the primary payer, transportation claims go to Modivcare, providers must follow Modivcare’s authorization rules, and trips Modivcare did not authorize are denied. Standing orders and facility trips

Dialysis centers, nursing homes, hospitals, and mental health facilities book rides through Modivcare’s Facility Department at 1-866-252-1566. Its Florida facility page sets these rules:

  • Standing orders cover a member with three or more trips a week to the same place. The facility faxes the form to 1-866-779-5242, and Modivcare recertifies every standing order each quarter.
  • Changes go through Modivcare. Providers work from the daily manifest, so a change a facility gives only to you can send the trip to another company.
  • Storms and holidays. A Hurricane and Holiday Schedule Change Form handles one-time changes to a standing order.
  • One-time trips can be faxed up to 30 days ahead, with 3 business days of notice for routine trips.
  • Preferred providers. A facility may ask for your company. Modivcare notes the request, but it assigns trips by routing rules that include provider compliance.
  • Lifts. A member who can walk but needs a wheelchair lift to board is booked at the ambulatory level, with a lift requested.

What to do first

  1. Find your region in the table above, and list the Modivcare programs that run there.
  2. Enroll with Florida Medicaid and finish owner screenings, since Modivcare’s plans check your AHCA agreement.
  3. Get your county permit if your garage is in Miami-Dade, Broward, or Palm Beach.
  4. Apply to Modivcare, and name any county on your list where its 2025 network had no vehicles.
