Humana Transportation Providers: Who Books Humana Medicare Advantage Rides and How to Get the Trips
Humana does not dispatch its Medicare Advantage rides. Customer Care confirms the benefit and sends the member to a ride vendor, and the vendor differs by market: SafeRide Health in several states since January 1, 2025, MTM Health, and Alivi for CarePlus plans in Florida. To become a Humana transportation provider, join the vendor that books Humana trips in your counties.
- Humana Customer Care checks the benefit and hands the member to a ride vendor, so the vendor is where you apply.
- For 2026, 318 of 886 Humana plans sold to individuals include rides, and 317 of those require the trip to be authorized.
- 294 Humana plans add unlimited rides for members with end-stage renal disease, which covers a dialysis schedule that a 24-trip limit cannot.
- Every CarePlus plan in Florida has rides, and its members book them in Alivi's CareTrip app or through Member Services.
- SafeRide also pays family drivers mileage for Humana members, but only for rides booked with SafeRide first.

For 2026, 318 Humana Medicare Advantage plans sold to individuals promise rides to plan-approved health care places. Humana’s plan documents do not send those rides straight to a transportation company. A member calls Humana, Humana points the member to its ride vendor, and the vendor books the trip with a company in its network. Your first job is to find out which vendor that is in the counties you serve.
Who books Humana Medicare Advantage rides
Humana’s 2026 evidence of coverage documents describe one path, plan after plan. The member calls Customer Care at 800-457-4708 (TTY 711). Customer Care confirms the benefit and guides the member to “the transportation provider,” and the member must schedule with that provider at least 72 hours (3 business days) before the appointment. The documents never name the vendor. Each vendor lists its Humana work on its own pages:
| Vendor | Humana work it shows, as of September 30, 2026 | Line for members |
|---|---|---|
| SafeRide Health | Humana Medicare Advantage members in several states since January 1, 2025, plus Humana’s gas mileage reimbursement for Medicare Advantage and Medicaid members | 877-841-3647, on its Humana member page |
| MTM Health | “Humana Medicare (national)” on the list of plans whose members use its MTM Link app | Humana Customer Care, 800-457-4708 |
| Alivi | CarePlus and Humana Medicare on its facility booking form, and CarePlus in its CareTrip app | CarePlus Member Services, 800-794-5907 |
| Modivcare | A Humana Medicare line (1-866-779-0567) and a Humana Group Retiree line (1-866-588-5121) on its Florida facilities page | The listed lines |
Treat the Modivcare listings with care. The same Florida page still lists a Cigna Healthcare Medicare Advantage line, and those plans, now sold as HealthSpring, moved their rides to MTM Health on March 1, 2026. Get written confirmation of any Humana work before you plan around it.
CarePlus rides in Florida go through Alivi
CarePlus Health Plans is Humana’s Florida Medicare Advantage company. Its October 22, 2025 announcement said every 2026 CarePlus plan provides transportation, with unlimited rides on its dual eligible plans, and the CMS file agrees: all 37 CarePlus plan options include rides. The 2026 CarePlus evidence of coverage tells members to book in the CareTrip app, which Alivi describes as its own member app, or through Member Services.
CarePlus trips come with their own rules:
- Members book at least 3 weekdays before the appointment.
- A companion may ride if the member tells the vendor ahead of time.
- Members who use a wheelchair tell the vendor when they book.
- Trips over 35 miles one way may need prior authorization.
- In the CareOne Plus H1019-001 point-of-service plan, a member who uses a ride company outside the network pays all charges.
The five CarePlus dual eligible plans have no trip limit. The other CarePlus plans allow 20, 26, or 50 one-way trips a year.
What Humana plans cover in 2026
CMS’s plan benefit file for 2026, dated October 1, 2025, shows how each Humana plan built its ride benefit. These counts cover the 886 Humana plan options sold to individuals, with employer plans left out.
| Humana plan type | Plans | With a ride benefit |
|---|---|---|
| Dual eligible special needs plans (D-SNPs) | 146 | 117 |
| Chronic condition special needs plans | 74 | 54 |
| Institutional special needs plans | 13 | 0 |
| Plans that are not special needs plans | 653 | 147 |
| All Humana plans sold to individuals | 886 | 318 |
HMO plans carry most of the rides: 202 of 356 Humana HMO options include them, against 89 of 464 PPO options. Among the 292 ride plans with a yearly limit, the median is 36 one-way trips, and 26 plans have no limit at all. Every ride plan lists car, wheelchair access vehicle, rideshare, and van as its ways to travel.
Limits and distance rules change from plan to plan, even in one state:
| 2026 plan | Area | Regular one-way trips a year | Distance rule |
|---|---|---|---|
| Humana Gold Plus SNP-DE H1036-324 and H5619-166 | Washington | 24 | 50 miles a trip |
| Humana Gold Plus SNP-DE H5619-167 | Washington, same counties | 24 | 75 miles a trip |
| Humana Gold Plus SNP-DE H4461-072 | Dallas-Fort Worth, Houston, and East Texas | 48 | 75 miles a trip |
| Humana Gold Plus H0028-029 | Corpus Christi area | 60 | No mile limit |
| Humana Gold Plus H1036-025 | Hernando, Hillsborough, Pasco, and Pinellas | 50 | No mile limit listed; some trips need prior authorization |
| Humana Gold Plus SNP-DE H1036-077A | Broward, Miami-Dade, and Palm Beach | No limit | No mile limit listed; some trips need prior authorization |
| CareOne Plus H1019-001 | Broward and Palm Beach | 50 | Over 35 miles one way may need prior authorization |
Florida is where Humana rides concentrate. It has 90 of the 318 ride plans, 37 of them CarePlus, followed by Texas (33), Louisiana (22), Kentucky (19), Mississippi (14), and Pennsylvania (13).
The second ride benefit for dialysis and cancer patients
Humana files a separate benefit, Uniform Flexibility Non-Emergency Medical Transportation, in 297 plans. It is aimed at members with end-stage renal disease. In 292 of those plans it also covers members with chronic kidney disease or cancer, and in 2 more chronic kidney disease alone. In 294 of them the trips have no limit, and in the other 3 members get 6 one-way trips a month. The Washington and Texas D-SNPs above give qualifying members unlimited one-way trips to plan-approved health care places, under the same mile cap as their regular rides, on top of the regular 24 or 48.
For a dialysis rider, this benefit matters more than the regular limit. A chair schedule of three days a week takes 312 one-way trips a year, far past any regular Humana limit short of unlimited. See dialysis transportation for how standing orders run.
How to become a Humana transportation provider
You apply to the vendor, which holds the Humana work. The steps:
- Find the vendor for your counties. Ask the dialysis centers, clinics, and senior living buildings you serve which number they call for Humana patients. In Florida, ask whether their patients are in CarePlus.
- Apply to SafeRide. Its form needs a real NPI, and an application left unfinished for 30 days starts over. SafeRide asks for $1 million general liability per occurrence, $2 million aggregate, $500,000 auto liability, and SafeRide named as additional insured. Each driver needs a national background check, a national sex offender registry check, a motor vehicle report, a pre-hire drug screen, first aid or CPR training, defensive driving, and wheelchair securement training.
- Apply to MTM Health on its service providers page. MTM says it answers quickly when it needs providers in your area and keeps your file when it does not.
- Contact Alivi for CarePlus work. Its transportation provider form asks for your state and counties, NPI, driver and vehicle counts, levels of service, and dispatch platform, and Alivi calls back within 2 business days.
- Set up the Medicare paperwork. Plan trips bring exclusion checks and long record keeping. See Medicare Advantage transportation and NEMT broker credentialing.
To stay in SafeRide’s network, you must keep a 100 percent ride response rate, 95 percent acceptance, 95 percent on-time performance, complaints under 1 percent, and fewer than 5 percent of accepted trips handed back.
Family drivers compete for Humana trips
SafeRide runs Humana’s gas mileage reimbursement for Medicare Advantage and Medicaid members. A caregiver, family member, neighbor, or the member can drive and be paid for the miles, once SafeRide approves the driver’s registration. SafeRide books those trips too: every claim needs a ride booked with SafeRide before the trip. So some Humana riders who could sit in your sedan will ride with family instead.
What a Humana trip asks of your dispatcher
- Notice. Members book at least 72 hours (3 business days) ahead, or 3 weekdays for CarePlus. Ask your vendor how far ahead it releases those trips to you.
- Approval. In CMS’s file, 317 of the 318 ride plans require authorization. Run only trips the vendor sends. If a Humana member calls you directly, send the member to the vendor.
- Counting. Humana counts one-way trips, so a round trip uses two. When a member runs out, the plan stops paying for rides, so a further trip is private pay unless Medicaid covers it. See private pay NEMT.
- Distance. Check the plan’s per-trip cap before you accept a long trip. In Washington, H1036-324 allows 50 miles a trip and H5619-167, sold in the same counties, allows 75.
Humana dual eligible members and Medicaid rides
Humana’s D-SNP members have Medicaid as well as Medicare. In 2026, 117 of its 146 D-SNPs include plan rides, most often with 36 or 24 one-way trips a year, and 23 with no limit. Rides that Medicaid covers for them run through the state’s Medicaid ride program or Medicaid health plan, so the same rider may reach you through two different vendors. See dual eligible, and for Florida Medicaid, Humana Healthy Horizons in Florida.

Frequently asked questions
How do I become a Humana transportation provider?
Join the ride vendor that books Humana trips in your area. Humana's 2026 evidence of coverage sends members from Customer Care to "the transportation provider" to book. SafeRide Health takes applications on its NEMT provider page, MTM Health on its service providers page, and Alivi, which books CarePlus rides in Florida, through its transportation provider form. Ask each one whether it needs Humana capacity in your counties before you finish credentialing.
Does Humana use SafeRide or Modivcare for Medicare Advantage rides?
It depends on the market. SafeRide says it has supported Humana's Medicare Advantage members in several states since January 1, 2025, with a member line of 877-841-3647. MTM Health lists Humana Medicare on its member app page. As of September 30, 2026, Modivcare's Florida facilities page still lists a Humana Medicare line, 1-866-779-0567. Ask the vendor to confirm in writing which Humana plans and counties it serves.
How many rides does Humana Medicare Advantage cover?
It depends on the plan. Of the 318 Humana plans with a 2026 ride benefit, 26 have no trip limit and the rest allow a set number of one-way trips a year, most often 24, 60, or 36. Some plans also cap each trip, such as 50 miles in the Washington dual eligible plan H1036-324 and 75 miles in H4461-072, which serves Dallas, Houston, and East Texas. A round trip counts as two trips.
Does Humana cover rides to dialysis?
Yes, in many plans. Besides the regular ride benefit, 297 Humana plans file a second benefit, Uniform Flexibility Non-Emergency Medical Transportation, for members with end-stage renal disease, and in most of them chronic kidney disease or cancer too. In 294 of those plans the trips are unlimited. Members book them like regular rides: at least 72 hours ahead in Humana plans, or 3 weekdays in CarePlus plans.
How far ahead do Humana members have to book a ride?
Humana's 2026 evidence of coverage requires members to schedule with the transportation provider at least 72 hours (3 business days) before the appointment. CarePlus plans in Florida ask for at least 3 weekdays. Some trips also need prior authorization, such as CarePlus trips over 35 miles one way.
Can a family member get paid to drive a Humana member?
Yes, through SafeRide's gas mileage reimbursement for Humana Medicare Advantage and Medicaid members. A caregiver, family member, neighbor, or the member can drive after registering the driver with SafeRide. Every claim needs a ride booked with SafeRide before the trip, and SafeRide says rates vary by state and health plan.
