Verida in Indiana: Traditional Medicaid Rides and Pay (2027)
Verida has arranged Indiana Traditional Medicaid (fee-for-service) rides in all 92 counties since June 1, 2018. To get its trips, enroll with Indiana Medicaid as a transportation provider, send Verida its Request for Qualifications and Vehicle List Form, and call your region: 765-602-6004 in the north or 317-931-9294 in the south. Verida pays from a rate agreement based on the state fee schedule.
- Every company that drives Verida trips must be enrolled with Indiana Medicaid first, so the IHCP enrollment comes before the Request for Qualifications.
- Members and hospitals can ask Verida for a preferred provider, so tell your regular riders and discharge planners once you are in the network.
- Space distance variance and incorrect mobility type were the top two reasons Verida denied Indiana claims in April and May 2026.
- Send trips back early: in May 2026, Verida reassigned 89 percent of trips returned more than 48 hours ahead, but only 77 percent of later ones.

Verida, Inc., the Villa Rica, Georgia company once called Southeastrans, is the Indiana Family and Social Services Administration’s broker for Traditional Medicaid rides. FSSA pays it a capitation rate for each eligible member, and Verida books each ride with a company in its network. Members of HIP, Hoosier Healthwise, Hoosier Care Connect, and PathWays ride through their health plan’s broker instead, mostly WellTrans. Who rides with Verida in Indiana
In May 2026, FSSA paid Verida for 127,920 fee-for-service members with the ride benefit, according to Verida’s monthly report under Senate Enrolled Act 480. The pool shrank when Indiana PathWays for Aging began on July 1, 2024: Verida’s members fell from 234,791 in June 2024 to 131,182 in July 2024. In August 2024, FSSA told its NEMT Commission which members stayed with Verida:
- Fee-for-service members 59 and younger.
- American Indian and Alaska Native members who opt out of managed care.
- Members with end-stage renal disease (ESRD) coverage.
- Members on waivers through the Division of Disability and Rehabilitative Services, and Traumatic Brain Injury waiver members.
- Members covered for breast and cervical cancer (MA-12), Room and Board (RCAP) members, and hospice members who opted out of PathWays.

Some fee-for-service trips never pass through Verida: stretcher, BLS, and ALS trips, every hospital-to-hospital trip, most nursing facility residents’ rides, and rides to 1915(c) waiver services. The state guide covers how those are booked. Two edges matter for Verida work. A waiver member riding to a service that is not a waiver service still goes through Verida, and so does a wheelchair van trip run by an ambulance company. When one member gets a Verida trip and a nonbrokered trip on the same day, bill them on two claims, one to each payer.
Where Verida’s Indiana trips are
On August 18, 2026, Verida gave the NEMT Commission its monthly averages for its second contract year, July 2025 through June 2026:

Trip volume fell about 9 percent from the first contract year. In June 2026, 28 more providers with 42 vehicles were in credentialing, and Verida named rising insurance and fuel costs, labor shortages, and operating costs as the industry’s challenges.

FSSA counts a vehicle in every county where it picks up, and the report adds 81 vehicles it lists as non-contracted, so the county counts total 1,464 against Verida’s 838 active vehicles. Marion carried more than two and a half times the statewide load per vehicle, and Lake and St. Joseph about twice. FSSA’s heat map tracks the same counts every month, including the counties where no Verida vehicle picked up at all, which the state guide lists.
How to become a Verida provider in Indiana
Every company that drives Verida trips must be enrolled with the Indiana Health Coverage Programs, usually as a common carrier (specialty 264 for ambulatory or 265 for wheelchair). In the first quarter of the contract year that began in July 2024, FSSA put Verida on a corrective action plan for paying providers outside the IHCP, which the state later closed. A rideshare company enrolled as specialty 267 and a family member driver enrolled as specialty 266 also contract with Verida after enrolling.
Once you are enrolled, Verida’s Indiana steps as of September 2026:
- Send the Request for Qualifications and Vehicle List Form with a driver list. Verida reviews whether your region needs another provider and calls you for an interview only if it does.
- Send your documents: a current business license, proof of liability insurance, driver credentials, and current criminal background checks, drug screens, and driving reports for every driver. Owners need current drug screens too.
- Pass the vehicle inspection. As Verida described it to the NEMT Commission in August 2024, the first inspection of each vehicle is a 72-point check with photos, recorded on a mobile app, and later inspections can come unannounced.
- Train your drivers. Verida’s August 2024 report said drivers train at onboarding and every year after, and the training includes PASS.
- Attend the owner orientation and sign. You sign the provider agreement and a Verida rate agreement. The state module gives new providers 60 days from signing a provider agreement to finish credentialing.

Expect inspectors. The contract measures FSSA reviewed for July 2024 through June 2025 require Verida to spot-check at least one twelfth of authorized vehicles every month without notice and to run at least 75 wheelchair securement inspections a quarter. Every driver who works in a quarter must meet the contract’s licensing and training rules, and one found out of compliance in a random audit counts against Verida. That year Verida ran 2,557 routine inspections, 1,342 spot inspections, and 391 securement inspections, all passed. Keep each driver file current.
How Verida books Indiana trips
Members book at least two business days ahead and up to 30 days ahead, by phone at 855-325-7586 (weekdays 8 a.m. to 6 p.m. Eastern) or on Verida’s member portal. Verida calls to confirm each ride at least 24 hours before pickup. A ride booked on shorter notice needs the healthcare provider to confirm the appointment. Verida treats transfers between facilities, such as a hospital to an extended care facility, as urgent trips, served within three hours of notice. See same-day NEMT trips.
Riders and hospitals can ask for you
The state module lets a fee-for-service member ask Verida to book rides with a preferred provider. The company must be enrolled with the IHCP, contracted with Verida or in credentialing, and able to run the level of service the member needs. Hospitals book discharges home on Verida’s Facility Dispatch Line, 888-822-6104, and Verida respects the hospital’s current preferred-provider relationships. Standing orders
Facilities request standing orders on Verida’s Indiana Standing Order Request Form, version dated May 13, 2026, faxed to 317-642-0913 within five business days of the first ride. The member needs two or more recurring appointments a week for at least six weeks. The facility recertifies dialysis standing orders every six months and all others every three months, or Verida cancels them. The facility line is 855-325-7588. A pharmacy stop on the way home from an appointment is not a separate trip. The member or your driver calls Verida to add it.
Some riders never reach a van. Verida gives bus passes or paratransit to members within half a mile of a transit line, and friends or relatives can drive members for gas money. From April to June 2026, those two options carried about 1,900 of roughly 27,500 trips a month.
What riders are told to expect
Verida’s Indiana rider guide (January 2024) and facility guide (April 2024) tell riders that drivers wear an ID badge and that vehicles carry the company’s name. Drivers wait only 10 minutes at pickup, and once a rider calls to say they are ready to go home, the company has one hour to arrive. A ride counts as on time if it arrives within 15 minutes of the appointment time. Riders call Where’s My Ride, 855-325-7586, option 2, after waiting 15 minutes past their pickup time.
The contract adds its own targets. In July 2024 through June 2025, Verida had to get 95 percent of return pickups done within one hour of the call and 90 percent of riders to appointments on time, whatever the traffic or weather. The same measures barred asking a rider to board before the scheduled pickup time and required companies to warn riders about late pickups and medical offices about late drop-offs. When you cannot run an assigned trip, send it back early. In May 2026, companies sent back 1,965 of 17,080 assigned trips, or 11.5 percent. Verida found a new provider for 89 percent of trips sent back more than 48 hours ahead and 77 percent of later ones. That month, 340 legs had no provider assigned, 66 were provider no-shows, and 234 were member no-shows.
What Verida pays in Indiana and why claims get denied
The state module bases Verida’s rate schedule on the IHCP Professional Fee Schedule. Each company signs a Verida rate agreement, and Verida may negotiate higher or special rates one company at a time. For 2026 the IHCP schedule sets a base rate of $31.79 for a wheelchair van trip (A0130) and $12.71 for an ambulatory trip (T2003), with mileage paid separately. When Verida pays
The state module says clean claims that reach Verida by Wednesday are paid within 14 days, by check or electronic funds transfer, with a remittance advice for each cycle. The contract measures for July 2024 through June 2025 asked Verida to pay or deny 98 percent of electronic clean claims within 21 calendar days and paper claims within 30, and to get 70 percent of claims filed electronically. Verida missed the electronic target in all four quarters, and in May 2026, 15,056 of 22,268 clean claims (68 percent) arrived electronically. For that month, Verida’s report listed averages of 11 days from the trip to the claim reaching Verida, 18 days from receipt to a decision, and 18 more days to the remittance date. The denial reasons
Verida denied 1,090 of 22,268 clean claims in May 2026, or 5.15 percent. The reasons, as its report to FSSA labels them:

April 2026 had the same top two, 444 and 239 claims, with a 6.39 percent denial rate. Most of these trace to the trip record. Run each trip in a vehicle and with a driver Verida has approved, bill the mobility type Verida scheduled, and record every time, odometer reading, and signature. Use a trip log and a claim denial log.
Rules only Verida claims have
- Medicare and Medicaid members. For a dual eligible rider on a Verida trip, send the claim straight to Verida with the Trip Leg ID from scheduling. Do not bill Medicare first.
- Prior authorization. Verida gets any authorization a brokered trip needs, such as an out-of-state trip. You never file one.
- Riders approved later. If you drove an applicant whose fee-for-service coverage was later made retroactive to the trip date, call Verida for claim instructions.
- 911 downgrades. An ambulance trip that turns out not to need life support, for a member outside a nursing facility, goes to Verida on a CMS-1500 with “911 Downgrade. Non-ALS/BLS Trip” in field 23. Send it through the Provider Portal or by mail to Claim Processing, Verida, 4751 Best Rd., Suite 300, Atlanta, GA 30337.

How FSSA grades Verida
FSSA withholds 3 percent of Verida’s capitation, and Verida earns it back by meeting performance measures in nine categories. The NEMT Commission votes on the payout once a year. In the year before, the provider services category covered claim payment times, the share of electronic claims, a no-provider-assigned rate of 5 percent or less, and a monthly regional gap report. The requests category measured whether riders got the rides they were scheduled for: Verida delivered 75.6 to 76.9 percent against a 90 percent target. As of December 2025, FSSA meets with Verida weekly and monthly, reviews its reports, and audits it on site, with a yearly on-site audit of program integrity and claims processing. A program integrity corrective action plan ran from November 18, 2024 to July 11, 2025. Verida reports how many rider complaints it acknowledges within one business day and closes within 15 business days: in May 2026 it logged four, all handled in time. Yes. Verida arranges rides only for Traditional Medicaid members. As of September 2026, Anthem, CareSource, and MHS members in HIP and Hoosier Healthwise ride through WellTrans, and UnitedHealthcare and Humana members ride through LCP Transportation. Each broker has its own contract, and all of them require your IHCP enrollment.
Does Verida arrange stretcher rides in Indiana?
No. Since July 1, 2023, a fee-for-service member who needs a stretcher, BLS, or ALS is booked directly with an IHCP-enrolled ambulance provider, which bills Indiana Medicaid. A wheelchair van trip run by an ambulance company still goes through Verida, and a 911 trip that turns out not to need life support is billed to Verida when the member does not live in a nursing facility.
How do I dispute a claim Verida denied or underpaid?
Start with an administrative review, requested after you sign in to the Verida Provider Portal at provider.verida.com. It covers claim questions, underpayments, and overpayments. If that does not settle it, send a written appeal to Claim Appeals, Verida Claims, 843 Dallas Highway, Villa Rica, GA 30180. The process applies even to companies without a Verida contract.
