MTM Health in Michigan: Health Plans, Trip Logs, and How to Join (2027)

In Michigan, MTM Health books rides only for Medicaid health plans. From October 1, 2026, those are Aetna Better Health of Michigan, Blue Cross Complete, HAP CareSource, and UnitedHealthcare Community Plan, with Molina switching to SafeRide Health. To join, get a CHAMPS ID and LARA certification first, because MTM voids Michigan applications that arrive without them.

  • MTM works only for health plans in Michigan, so every MTM trip there is a plan member, never a fee-for-service rider.
  • Molina's rides move from MTM to SafeRide Health on October 1, 2026, so a company that drove Molina members for MTM needs a SafeRide agreement to keep them.

Michigan gives MTM Health no fee-for-service work. Every MTM trip in the state comes from a Medicaid health plan that hired MTM to run its ride benefit, while members without a plan ride through their local MDHHS office or, in Wayne, Oakland, and Macomb counties, through Modivcare. Which Michigan health plans send rides to MTM

MDHHS keeps one contact sheet for the health plans’ ride programs. The version revised for October 1, 2026 names MTM for four of Michigan’s nine Medicaid health plans:

Aetna’s Michigan trip log lists a second MTM line, 888-513-1612, for Aetna’s Michigan HIDE plan, which serves members who have both Medicare and Medicaid.

Molina moves to SafeRide on October 1, 2026

Molina Healthcare of Michigan leaves MTM on October 1, 2026. The same MDHHS sheet lists SafeRide Health as Molina’s ride vendor and sends members to SafeRide directly at 888-616-4842 from October 1, 2026. The sheet’s May 14, 2026 version gave that same number for MTM. If you drove Molina members for MTM, apply to SafeRide to keep them. The other plans use other arrangements. Meridian, Priority Health, and now Molina ride through SafeRide Health, McLaren rides through Modivcare, and Upper Peninsula Health Plan pays members back for mileage and runs its own volunteer drivers, buses, and taxis.

The rides plan members can book

The Medicaid Provider Manual dated October 1, 2026 makes each plan responsible for its members’ rides to all Medicaid services, Healthy Kids Dental visits included. That policy dates from MDHHS Letter 24-36, issued July 11, 2024 and effective October 1, 2024. Blue Cross Complete’s March 2026 provider newsletter lists its members’ rides to specialty mental health services and substance use disorder treatment under it.

Some plans go further. As of September 2026, HAP CareSource’s member page offers rides to community and religious events and CPR classes, plus five round trips a month to food banks, food pantries, and grocery order pickups within 25 miles. It rules out personal errands such as hair appointments and daycare pickups. Expect those trips on your manifest if you drive HAP CareSource members.

Where MTM’s Michigan plans ride

MDHHS awarded the current health plan contracts by the state’s 10 prosperity regions. They took effect October 1, 2024, for five years with three one-year extension options. With Molina gone, MTM’s plans sit here:

Three things follow for a small company:

  • Southeast and Detroit Metro. All four MTM plans serve these nine counties, so an MTM agreement there opens the most plan members.
  • Lansing and the southwest. Blue Cross Complete is not awarded in the South Central or Southwest regions, so MTM’s trips around Lansing, Kalamazoo, and Battle Creek come from Aetna, UnitedHealthcare, and, around Lansing, HAP CareSource.
  • The north and mid-Michigan. In the Northwest, Northeast, and East Central regions, Blue Cross Complete is MTM’s only plan. The Blue Cross Complete guide has its rules. In the Upper Peninsula, look to Upper Peninsula Health Plan instead.

What each plan’s rules mean for your drivers

MTM books the trip, but the plan sets many of the rules your driver meets at the door. From the MDHHS sheet revised for October 1, 2026:

  • Blue Cross Complete. Members bring their own car seats. Extra riders come along when the member asks at booking. Urgent trips and hospital discharges run the same day, after-hours discharges and trip recovery run around the clock, and a lead screening counts as urgent. When a late ride makes a member miss a visit, the plan waives its 48-hour rule and sends the member to the next open appointment, even the same day.
  • Aetna Better Health. Escorts ride, up to the vehicle’s capacity once car and booster seats are counted. When a foster child’s visit needs both the foster parent and the birth parent, the plan authorizes MTM to carry both. MTM tries to fit in urgent trips booked with less than three days’ notice. When a ride runs late, the vendor is to tell the member, and MTM tries to rebook the trip that day or when the member reschedules.
  • HAP CareSource. One adult or any number of children can ride along. If a ride fails and the member may miss the visit, the driver reports it to MTM so another driver can be sent.
  • UnitedHealthcare. Only the member rides, unless the member is a child or an adult who needs help. MTM fits in same-day rides for Children’s Special Health Care Services (CSHCS) members, and urgent rides go out right away when a driver is free, with no guarantee.

How to join MTM in Michigan

MTM’s 2025 provider application sets a Michigan rule the other steps depend on: a valid CHAMPS ID and certification through LARA are required to proceed, and applications without them are voided. Do the state steps first.

  1. Get your vehicle authority. LARA licenses limousine carriers, whose vehicles seat 8 or fewer, driver included. As of September 2026, one vehicle costs $125 the first year and $100 to renew each August 31. MDOT says a non-emergency vehicle that carries 9 or more, driver included, falls under the Motor Bus Transportation Act, and a vehicle that can seat 9 stays under it even if you let only 8 ride, unless the seats are physically removed. MTM’s form names LARA, so if your fleet runs on MDOT authority, ask MTM how to show it.
  2. Enroll in CHAMPS. MDHHS enrolls NEMT companies as atypical providers, organizations that do not provide health care and may not have an NPI. The Michigan guide walks through the vendor registration, MILogin, and screening steps. The Provider Enrollment line is 800-292-2550.
  3. Apply on MTM’s online form. Enter your CHAMPS ID in the Michigan field, the counties you serve on a regular basis, and how many ambulatory, wheelchair, and stretcher vehicles you run.
  4. Work through MTM’s checkpoints. A network representative interviews you, then come the contract, credentialing, training, and a first trip. MTM moves ahead where it needs providers and keeps other applications on file.

Your drivers answer to the state and to MTM. The Medicaid Provider Manual dated October 1, 2026 sets the floor for commercial providers: drivers at least 18, able to read and communicate well in English, and licensed for the vehicle class. MDHHS can deny or revoke enrollment over a restricted or suspended license. MTM’s agreement adds its own rules, such as the minimum driver age of 21 in its standard agreement, which the MTM Health guide covers. MTM sets your rate in its agreement. Plan trips are not paid from the MDHHS fee-for-service schedule, which since July 1, 2026 pays up to $0.76 a mile, and $35 a round trip plus $0.76 a mile in a wheelchair lift or Medi-Van vehicle, on rides the local offices arrange. That schedule is still a useful yardstick when you negotiate a broker rate.

The MTM trip logs Michigan members use

The MTM trip log Michigan residents search for is a member form. Members who drive themselves, or ride with a friend or relative, use it to get gas money back. MTM’s mileage page lists four Michigan logs as of September 2026:

On every log, the member calls for a trip number on or before the appointment day and has any health care professional at the visit sign, such as a nurse, therapist, physician assistant, or nurse practitioner. Logs are due within 365 days of the first appointment and go to MTM Health, Attention: Trip Logs, 16 Hawk Ridge Dr., Lake St. Louis, MO 63367, by fax to 888-513-1610, or to payme@mtm-inc.net.

The attestation is an MDHHS form under the Consolidated Appropriations Act of 2021. The driver signs that they are not excluded from federal health programs, hold a valid license, have no felony controlled-substance conviction after August 21, 1996, and have had no more than two moving violations and no OWI or DUI in the past two years.

Your company’s trips never go on these logs. They are recorded in the MTM Link driver app or in dispatch software MTM has approved, which the MTM Health guide explains. Keep your own trip log as a check against it.

  1. Find your prosperity region in the table above and list the MTM plans that serve it.
  2. Get your LARA or MDOT authority and your CHAMPS ID, since MTM voids a Michigan application without them.
  3. Apply to MTM with every county you can cover, and ask the network representative which plans need providers there.
