Modivcare Facility Portal in 2027: TripCare Sign-Up, Facility Lines, and Discharges by State

Overview
Modivcare's facility portal is TripCare, at tripcare.modivcare.com. Hospitals, clinics, dialysis centers, and nursing homes use it to book rides and standing orders, upload forms, track trips, and cancel rides up to the day before. For a login, use the Sign up for TripCare button on your state's Modivcare facility page. A few states also want a signed paper form.
- TripCare is the facility portal. Every Modivcare state page has the same Sign up for TripCare button, and older pages call it the Facility Services website.
- The sign-up button opens an online form for facility staff only, and Modivcare says it replies within 3 business days. West Virginia and California also ask for a faxed paper form.
- Same-day and urgent rides still go by phone in several states, so keep the facility line next to the portal.
- A facility can name a preferred ride company in TripCare or on the phone, but Modivcare only notes it and assigns trips by its own routing rules.
- Some Modivcare pages still show plans that left, such as HealthSpring, so check the member's plan before you call a listed line.
Clinics, dialysis centers, nursing homes, and hospitals book Modivcare rides for their patients through their own channel: TripCare, the facility portal, or a facility line in their state. This page covers how a facility gets a login, which line to call in each state, how fast discharges and late rides are handled, and how those trips reach a ride company.
What the Modivcare facility portal does
TripCare is the web portal where a facility requests and manages rides for the members it serves. Modivcare’s login page says a facility sees all the trips scheduled for it there and can book more. Older state pages call the same service the Facility Services website.
Modivcare’s TripCare Resource Center (updated April 18, 2024) lists what a facility can do in it:
- Request single trips and standing orders, such as dialysis three days a week.
- Upload the forms a ride needs, such as a medical necessity form, and submit them right away.
- Search requests and watch their status, then edit or withdraw them.
- Recertify, update, or end standing orders.
- See the details of every scheduled trip, and cancel a trip up until the day before.
- Send Modivcare an attendance log online.
TripCare also has Live Chat. Modivcare’s tips page says it can confirm a member’s eligibility and plan, make same-day changes and cancellations, and tell the facility where the vehicle is and when it will arrive. A “My Members” list keeps a scheduler’s regular members in one view, and coworkers can book and cancel for the facility without being added to it.
Phones still matter. Modivcare’s Florida facility instructions (July 2022) say urgent same-day trips still need a phone call, and South Carolina wants urgent care rides requested on its Facility Assistance Line.
How a facility gets a TripCare login
Every one of Modivcare’s 37 state facility pages carries the same Sign up for TripCare button. It opens an online registration form for hospital, clinic, dialysis center, and medical office staff only. Members, patients, and caregivers who fill it out are sent to Modivcare’s member portal instead.
- Find your state’s page at modivcare.com/facilities and use Sign up for TripCare.
- Give the facility’s details. The form asks for the facility name, address, phone, fax, Medicaid provider number or NPI, and the user’s name, email, and job title. It also asks what you need: a new administrator, removing one, changing an email address, or reactivating an account.
- Agree to the access rules and sign. You check a box agreeing to follow HIPAA, share access only with staff who need it, remove people who leave, and never share a password. You type your name as your signature. Modivcare’s confirmation says you will hear back within 3 business days with the next steps.
- Look for a paper form on your state’s page. As of October 6, 2026, 15 state pages post a Medical Provider EDI form, including West Virginia, Florida, Utah, Michigan, Maine, Delaware, Texas, and Pennsylvania. West Virginia’s page calls it the required registration form for TripCare logins, and California’s page says to fax it to 1-877-601-0535 to get login credentials. The paper form asks the same questions and adds a signature from a witness at your facility. If your state posts it, ask your facility line whether the online form is enough.
- Add your staff. Modivcare’s paper form says each facility gets two administrator logins, or more on request, and that the administrators create logins for coworkers and remove them when they leave. Florida’s version of the form also adds or deactivates an administrator and resets a password.
Some states use their own step instead. Oklahoma asks every facility that needs standing orders to send a Facility Registration Form first, with its NPI, Medicaid provider number, hours, observed holidays, and a contact person. Delaware’s Facility Department sets up user names at 866-469-2824, and South Carolina’s Facility Assistance Line sends a user form and manual on request. HMSA facilities in Hawaii have used TripCare for every inter-island flight request since July 1, 2025.

Its training videos cover access, navigation, attendance, single trips, and standing orders, and its Resource Center says your Facility Liaison can invite you to weekly virtual training sessions and office hours.
Modivcare facility lines by state
When a facility cannot use TripCare, it calls the facility line for its state. This table covers the 16 states where Modivcare arranges Medicaid rides and prints a facility line on its state page or forms, as of October 6, 2026. Two programs have no separate facility line on Modivcare’s pages. In Mississippi, medical offices book fee-for-service rides on the reservation line, 1-866-331-6004. Modivcare’s Pennsylvania page, for Philadelphia’s program, gives facilities the TripCare letter and the EDI form. ʻOhana Health Plan leaves QUEST on December 31, 2026, so its Hawaii line ends with it.
Check the plan before you call a line from a Modivcare page, because some pages still show plans that left. Twenty-two of the 37 state pages still list HealthSpring Medicare Advantage at 1-866-214-5126, but HealthSpring moved its Medicare Advantage rides to MTM Health on March 1, 2026 (the HealthSpring switch). Virginia’s page still lists fee-for-service Medicaid, which MTM Health took over on October 1, 2026.
Hospital discharges through the facility side
Discharges are the one trip a facility cannot plan days ahead, so most programs take them at any hour and set a pickup window. In most states the clock starts once the discharge is arranged, so call as soon as the patient has a discharge order. - New Jersey. Hospitals call 866-527-9945 and pick the discharge prompt. No medical necessity form is needed, and the provider has 3 hours to respond and pick up.
- Oklahoma. Hospital staff such as a discharge planner, social worker, or nurse call 877-404-4500, any hour, ideally 7 a.m. to 6 p.m. when known ahead. The provider has up to 3 hours.
- South Carolina. Only hospital staff book, on 866-420-6231 at any hour. The provider has 30 minutes to 3 hours.
- Virginia. Hospital staff call 866-679-6330. Modivcare has 3 hours from the call to assign the trip and pick up, and it verifies the discharge with the hospital.
- Michigan. Hospitals call 866-569-1902 and pick the discharge prompt. The provider has 3 hours.
- Delaware. Discharges are taken at any hour, with a goal of a ride within 3 hours. A patient cannot call in their own discharge.
- Arkansas, Little Rock area. A doctor, nurse, or social worker calls the reservation line, which is 888-833-4135 in Pulaski, Faulkner, and Lonoke counties (the state calls it Region G) on the state’s page. Once a ride is assigned, it usually arrives within 3 hours. Call Saturday discharges in the Friday before.
- New Mexico and Hawaii ʻOhana. Hospitals call the facility line, and the provider has up to 4 hours.
- Hawaii HMSA. Hospitals book in TripCare’s Request Trip tab. Ambulatory and wheelchair rides are booked on the day of discharge, gurney and bariatric rides may be booked a day ahead, and Modivcare has a 4-hour window.
- California. Hospital staff call the member’s plan line at any hour, at least 2 hours ahead when possible.
When the ride is late
Each state has a Ride Assist or Where’s My Ride line that both members and facilities may call. The South Carolina, California, and Mississippi pages say to call once a ride is more than 15 minutes late. The same line sends a will-call return when an appointment ends at an unknown time.

Other states list a late-ride line without a will-call window. Delaware’s is 866-896-7211, Mississippi’s 1-866-334-3794 in the north and 1-866-331-6006 in the south, and Florida’s fee-for-service line 1-866-726-1458. South Carolina has one per region: 866-910-7689, 866-445-9962, and 866-445-9964. Montana’s Ride Assist option is on the member line, (800) 292-7114. Michigan’s page sends callers to the ride line for their plan, which is 866-569-1902 for Medicaid. Hawaii’s page lists 1-866-481-9699 for ʻOhana Ride Assist at the top and 866-475-5748 in its FAQ, so try the second if the first does not connect.
How facility-booked trips reach your company
A trip a facility books in TripCare, by fax, or by phone lands in Modivcare’s system. Modivcare then sends it to a ride company on that company’s daily manifest. A facility can name a company it prefers, and the Oklahoma, Florida, Hawaii, and New Mexico pages say Modivcare notes the request. Trips are still assigned by routing criteria that include provider compliance, with no guarantee. Delaware’s and Virginia’s pages go further: facilities there have no freedom of choice of provider. The portal also shapes how your service looks to the facility. Modivcare’s new-features page (updated April 18, 2024) says TripCare records pickup phone numbers so your driver can find the member and send an arrival time. Live Chat can tell the scheduler where your vehicle is and when it will arrive, so a late van is visible to the facility before anyone calls you. Facilities also send Modivcare attendance logs for the members they serve.
To win and keep facility work in a Modivcare state:
- Join the network in that state first. The Modivcare broker guide covers the application, and the state’s broker page covers local rules.
- Visit the schedulers who book most. Where a facility may name a company, dialysis social workers, nursing home staff, and hospital case managers decide whether yours goes in the preferred provider field. Delaware and Virginia facilities cannot.
- Send every change back to Modivcare. When a scheduler tells your driver about a new time, ask them to change it in TripCare or on the facility line.
- Plan for the discharge clock. Most of these states give 3 or 4 hours, so keep a vehicle free that can take a same-day discharge.
- Protect your compliance record. Modivcare’s routing criteria include provider compliance, so it affects whether a facility’s request for your company turns into trips.
