How to Become an LCP Transportation Provider in Indiana in 2027

Overview

LCP Transportation, LLC is the Indianapolis ride broker for UnitedHealthcare's Hoosier Care Connect and PathWays members, Humana's PathWays members, and routine rides on UnitedHealthcare's Indiana Dual Complete plans. To join, enroll with Indiana Medicaid, apply on LCP's Transportation Provider Network page, and send its July 2026 credentialing file: your IHCP letter, operating authority, an insurance certificate naming LCP, and driver checks and training.

LCP Transportation, LLC is an Indianapolis company, established in 1998, that runs ride programs for Medicaid health plans. It takes member calls at one statewide call center and sends trips to transportation companies it calls vendors, which it works with in designated service areas. LCP also runs its own drivers. Its company page says it covers most regular trips, and urgent trips in denser urban areas, with a mix of vendors and its own drivers. It keeps some after-hours and long-distance rural trips for its own drivers and uses vendors for regular local trips in sparsely populated rural areas. It can also offer bus passes and mileage reimbursement in some metropolitan areas when a plan wants that.

Which Indiana plans use LCP

As of October 2026, Indiana’s plan comparison pages list LCP for three Medicaid plan programs. UnitedHealthcare’s 2024 provider manuals, Humana’s ride page (April 7, 2026), and Humana’s PathWays handbook (May 2026) name LCP too, and LCP’s March 12, 2021 letter to its vendors dates its Hoosier Care Connect work to April 1, 2021. Indiana’s August 2026 enrollment report counts their members:

UnitedHealthcare’s 2026 enrollment guides for two Medicare plans for people with both Medicare and Medicaid also name LCP for routine rides to approved places such as medical appointments, gyms, and pharmacies: 24 one-way trips on Dual Complete IN-S002 and 36 on PathWays Dual Care IN-S1.

Other Indiana Medicaid riders go through other brokers. Since January 1, 2026, WellTrans books HIP and Hoosier Healthwise rides for Anthem, CareSource, and MHS (IHCP bulletin BT202608), and the state lists WellTrans for Anthem and MHS in Hoosier Care Connect and for Anthem in PathWays. Verida books Traditional Medicaid rides. The 2026 WellTrans change explains how MDwise’s exit reshaped the map. Humana’s handbook sends its members’ Medicare rides to a different company, so a dual member’s Medicare and Medicaid trips may come from two brokers.

How many riders LCP’s plans have

Together those three programs had 79,317 members in August 2026. That is 63 percent of PathWays’ 115,131 members but about 8 percent of Hoosier Care Connect’s 80,153.

PathWays covers Medicaid members 60 and older in a full-coverage aged, blind, or disabled category, including nursing facility residents and people on the Aged and Disabled waiver. So more than 9 in 10 of the members in LCP’s three programs are 60 or older, and some live in nursing facilities or get waiver care at home. Marion County had the most members in LCP’s three programs (15,203), then Lake (5,923), Allen (5,480), and St. Joseph (3,083). Because LCP mixes its own drivers with vendors, ask vendor relations which counties and trip types need vendors before you add a van for LCP work.

How to join LCP’s network

  1. Enroll with Indiana Medicaid. The IHCP Provider Enrollment module (version 7.1, September 8, 2026) requires transportation providers to enroll with the IHCP and also contract with the plan’s transportation broker to carry managed care members. The Indiana guide covers the common carrier specialties, the $50,000 surety bond, and Motor Carrier Services authority.
  2. Send the online application. The Apply Now button on LCP’s Transportation Provider Network page opens its Transportation Team Application. It asks for your company and contact details, how many vehicles you run in each class (passenger with capacity 4, van with capacity 6, lift with capacity 2, stretcher with capacity 1), your home county, the other counties you serve, and whether you drive out of state.
  3. Build the credentialing file from the checklist in the next section.
  4. Self-inspect each vehicle. The checklist calls for self-inspections on LCP’s detailed checklist with video, handled as a separate process. LCP’s company page says it also checks vendors through periodic vehicle and on-site inspections.
  5. Sign the NEMT agreement. LCP sends it, and you initial and sign it.
  6. Set up payment and the portal. Send a W-9 and a direct deposit form, then log in to LCP’s vendor portal for your schedules.

What LCP’s vendor checklist asks for

LCP’s New Vendor Credentialing Checklist (file dated July 7, 2026) asks for three groups of items.

Company and insurance

  • Licenses. Your IHCP enrollment letter and IHCP provider profile, and a transportation authority certificate, certificate of public convenience and necessity, or taxi license.
  • Forms. A vendor profile sheet, disclosure of ownership, W-9, and direct deposit form.
  • Liability certificate. An ACORD certificate of liability insurance with LCP Transportation, LLC as additional insured. The Description of Operations box must read: “LCP Transportation, LLC. is named as an additional insured. Thirty (30) day notice of cancellation will be given for any reason including non-payment of premiums.”
  • Workers’ compensation. A clearance certificate if your insurance does not cover it.
  • Each vehicle. Registration and proof of insurance, in your driver and vehicle logs.

The checklist prints LCP’s address for the certificate as 4310 Guion Road, Indianapolis, IN 46236, while LCP’s website gives ZIP code 46254, so ask vendor relations which to use before your agent issues the certificate of insurance.

Owners and drivers

  • A national criminal background check, a 10-panel drug screen, and a motor vehicle report, each less than six months old, for owners and drivers.
  • A copy of each driver’s license, and a sex offender registry check, which can be a line on the background check.
  • An E-Verify case report for all drivers and employees.

Training

  • Every employee and driver. HIPAA, fraud, waste, and abuse (FWA), culturally and linguistically appropriate services (CLAS), business ethics, information management confidentiality, and code of conduct. Also PASS, the Passenger Assistance Safety and Sensitivity course of the Community Transportation Association of America.
  • Every driver. First aid and CPR certification. See CPR requirements.
  • Drivers only. Wheelchair securement, through two-day PASS training or Q’Straint training. See wheelchair securement.

How LCP sends trips and pays

  • Trips. Vendors log in to LCP’s provider portal to see routes, passenger pickup times, and schedules, and to get invoices.
  • Claims. UnitedHealthcare’s Hoosier Care Connect provider manual (2024 edition) says the plan contracts with LCP for NEMT and tells providers to submit claims to LCP for adjudication. Non-emergency ambulance trips for those members are billed to UnitedHealthcare instead, and its PathWays manual says those members get non-emergency stretcher and ambulance rides through the plan.
  • Vehicles. Both UnitedHealthcare manuals say rides go by taxi, van, bus, or public transit, depending on the member’s medical needs, with wheelchair service when medically necessary.
  • Notice. LCP’s FAQ asks members for two business days. UnitedHealthcare’s manuals ask for at least 48 hours, with requests taken 8 a.m. to 8 p.m. Eastern, Monday to Friday.
  • Humana riders. Humana’s PathWays handbook (May 2026) asks members to book two business days ahead and cancel at least two hours before pickup, and tells them the driver waits only 30 minutes at the curb. Members or their doctor’s office call for the ride home, naming any pharmacy stop when they book it. Humana’s ride page (April 7, 2026) gives other times: 48 hours to book, and 24 hours before the appointment to cancel.
  • Escorts and changes. LCP allows one escort per rider. Members can change trips in their online account unless the trip is less than 72 hours away.
  • No-shows and disputes. Report no-shows and courtesy calls on LCP’s vendor and driver line. Claim disputes, appeals, and adjudication questions go to vendor claims by email.
