Molina Medicare Transportation: Ride Lines, 12-Trip Plans, and Card Allowances in Molina Dual Plans

Molina Medicare transportation comes through its dual eligible plans, and in 2026 it works three ways. Complete Care Plus plans in Illinois and Bexar County, Texas book medical rides through a ride line. Plans in Imperial County, California, New Mexico, Michigan, and 82 other Texas counties give 12 one-way trips a year. Plans in Idaho, Mississippi, Nebraska, Nevada, New York, and Utah give a monthly card allowance for rides.

Molina’s Medicare business is almost all dual eligible plans. For 2026, 55 of its 57 Medicare plan options sold to individuals are dual special needs plans, sold as Molina Medicare Complete Care, Complete Care Plus, and Complete Care Select, and under the Passport Advantage name in Kentucky, My Choice Wisconsin, and Senior Whole Health in New York and Massachusetts. So nearly every Molina Medicare rider also has Medicaid. What the plan itself pays for, though, changes from one plan to the next.

What Molina’s 2026 Medicare plans file for rides

CMS’s 2026 plan benefit files show 40 Molina plan options with a supplemental ride benefit, 39 of them dual plans, in 16 states. All 40 charge $0, need no referral, and list taxi, rideshare, bus, van, and medical transport. Thirty-five are filed with no trip limit and five with 12 one-way trips a year, and four of those five require authorization.

“No limit” can mean a card, not a trip count

In the no-limit plans below, the 2026 evidence of coverage turns the benefit into money on a prepaid card. The amount is a combined allowance shared with over-the-counter items, and in the Idaho and New York plans with food and utility help too, so money spent in the store is gone for rides. Unused money expires at the end of each month.

Members spend it on ride-hailing apps, taxis, bus passes, and rail cards, and the documents tell them to ask a dialysis center or therapy clinic about cheaper transportation before booking. The member buys those rides directly, so a van company’s steady work with these members comes from their Medicaid rides. Complete Care Plus: plans with their own ride line

In Illinois and Texas, Complete Care Plus covers Medicare and Medicaid in one plan, and its handbooks book medical rides the way a Medicaid plan does.

Illinois

Molina Medicare Complete Care Plus is sold as five Illinois plans: H3093-004 in Cook County, -005 in DuPage, Grundy, Kane, Kankakee, Kendall, Lake, McHenry, and Will, and -001, -002, and -003 across northwest, central, and southern Illinois, including Peoria, Sangamon, and St. Clair counties. The 2026 evidence of coverage for -001, -002, -003, and -005 sends members to the ride assist line, (844) 644-6353, at least 3 days before an appointment whenever possible. Hospital discharge rides can be booked with three hours’ notice, members can add a pharmacy stop on the way home if they call before leaving the office, and gas mileage reimbursement is available. Nonemergency ground ambulance trips are carved out of Medicaid managed care, and ambulance companies bill HFS directly.

On top of the ride line, those plans load a monthly card for rides to any health-related place: $174 in H3093-001, $128 in -002, $188 in -003, and $132 in -005, shared with over-the-counter items. Members who qualify for extra help for chronic illness also get 24 one-way trips a year to non-medical places, booked with the vendor at least 2 days ahead.

HFS’s June 2026 list of dual plan subcontractors names MTM Health for Complete Care Plus rides at 844-644-6353. Molina’s HealthChoice Illinois Medicaid plan uses MTM too, on a different line, 844-644-6354, according to the October 2026 HFS list. Bexar County, Texas

Complete Care Plus H6515-001 serves Bexar County, and its 2026 handbook covers STAR+PLUS transportation: curb-to-curb rides in buses, vans, and sedans, including wheelchair-accessible vehicles, transit passes, airline tickets, mileage for a friend or relative who drives, and an attendant’s ride. Nursing facility residents ride only to dialysis or on discharge to a lower level of care. Members book with Access2Care at (866) 462-4856, 24 hours a day, at least two business days ahead, and five days ahead for long-distance trips.

A Molina provider notice moved its Texas Medicaid rides from Access2Care and MTM to SafeRide Health on April 1, 2026. Medicaid members kept their ride line, (866) 462-4857, one digit off the Complete Care Plus number, and SafeRide asks for 48 hours’ notice. The notice speaks only of Medicaid members, so ask both MTM and SafeRide which one books Complete Care Plus riders in your county. The Medicaid side is on Molina Healthcare of Texas. Complete Care Plus plans also cover Dallas (H6515-002), El Paso (-003), Harris (-004), and Hidalgo (-005) counties, and their CMS filings cap the Medicare ride benefit at $130, $100, $75, and $170 a month.

Plans with 12 trips a year

Complete Care H3038-002 in Imperial County, California and H8845-007 in 25 New Mexico counties, including Bernalillo, Dona Ana, and Santa Fe, give 12 one-way trips a year to plan-approved places only. Their 2026 evidence of coverage sets rules your dispatcher will see:

  • Counting. Every leg is a trip. A pharmacy stop right after an appointment is free if the vendor knows in advance, and a separate pharmacy run counts as two.
  • Distance. A one-way trip over 100 miles needs prior authorization.
  • Booking. At least 2 days and at most 30 days ahead. Repeat trips for chemotherapy, radiation, physical therapy, or dialysis can be booked 90 days at a time.
  • Timing. The driver may arrive 15 minutes early or late. After a will-call trip request, the driver should pick up within 60 minutes, and a member left waiting over 15 minutes calls the vendor, not the driver.
  • Cancelling. Members must cancel at least 24 hours ahead. A ride not cancelled before the driver is dispatched counts against the 12. See late cancellation.
  • Vehicles. A sedan, van, taxi, or rideshare for members who can walk, and a wheelchair lift vehicle for limited mobility. Members bring their own wheelchair or cane.
  • Where to book. The vendor answers around the clock for discharges and ride status, and the California document points members to the Access2Care app.

Twelve trips cover only two weeks of a three-times-a-week dialysis schedule, so those riders go back to Medicaid for the rest of the year. The same limit applies to Complete Care H7678-006 in 82 Texas counties and H5926-001 in 54 Michigan counties, according to their CMS filings.

How to get Molina Medicare trips

  1. Join MTM Health. MTM’s MTM Link page lists Molina Medicare as a national client. Access2Care, named in several 2026 Molina documents, became part of MTM when MTM closed its purchase from Global Medical Response, in a release dated October 8, 2024, and MTM says it keeps working with Access2Care’s transportation providers. Apply on MTM’s service providers page; see MTM Health.
  2. Enroll in Medicaid in your state. Almost every Molina Medicare member has Medicaid, and a plan with a card allowance or 12 trips leaves most van trips to Medicaid. See how to contract with Medicaid health plans.
  3. Add the state’s Medicaid vendor for Molina. MTM Link lists Molina Medicaid in Iowa, Illinois, Florida, Michigan, Ohio, and South Carolina. SafeRide Health has member pages for Molina Healthcare of Texas and of Michigan; see Molina Michigan transportation and Molina Ohio transportation.

  5. Check the plan behind each rider. Even inside one state the benefit differs: in Texas, Complete Care Plus in Bexar County books rides through Access2Care, while Complete Care H7678-006 gives 12 trips a year in 82 other counties. Benefits and vendors can change each January 1.
