Blue Cross and CountyCare Leave Illinois MLTSS in 2027, and Humana Joins
Blue Cross and CountyCare leave Illinois MLTSS (MDSP) after Dec. 31, 2026, and Humana joins. Book each rider's January trips under the new plan.

On October 13, 2026, Blue Cross and Blue Shield of Illinois told providers that Managed Long-Term Services and Supports (MLTSS) moves out of HealthChoice Illinois on January 1, 2027, and that it stops covering its MLTSS members that day. Client Enrollment Services, the enrollment office of the Illinois Department of Healthcare and Family Services (HFS), says CountyCare leaves too. Neither plan is part of the program, which it calls the MLTSS Duals Specialty Program (MDSP), after December 31, 2026. If you drive nursing home, supportive living, or waiver riders for either plan, their January trips come through a new plan.
Who has to change plans
MLTSS members have both Medicare and Medicaid, so they are dual eligible. They live in a nursing home or supportive living facility, or they get waiver services at home, such as the Aging or Physically Disabled waivers.
- Blue Cross and CountyCare MDSP members. Neither plan takes new MDSP members. Current members keep their plan through December 31, 2026, and must choose a new one by that day for a January 1 start. Blue Cross says HFS will reassign each of its MLTSS members to one of the approved plans.
- CountyCare riders all live in Cook County, the only county CountyCare serves.
- Not changing. Blue Cross members who get waiver services but are not in MLTSS stay in Blue Cross Community Health Plans (BCCHP).
Members choose by calling Client Enrollment Services at 1-877-912-8880 (TTY 1-866-565-8576), weekdays 8 a.m. to 6 p.m.
The four MDSP plans and their ride lines
HFS’s October 2026 MDSP comparison chart lists four plans, and every one covers non-emergency transportation. The same four companies run Illinois’s fully integrated dual plans (FIDE SNPs). That fits the Blue Cross notice: MLTSS moves to plans run by FIDE SNP sponsors. HFS’s FIDE SNP model contract also says the state plans to add MLTSS to it in 2027.
| MDSP plan | Ride line on the chart | Broker for the company’s other Illinois plans |
|---|---|---|
| Aetna | None listed | Modivcare (HealthChoice), MTM (FIDE SNP) |
| Humana, from January 1, 2027 | App or 1-855-253-6867 | MTM (FIDE SNP, same number) |
| Meridian | 866-796-1165 | MTM (HealthChoice, same number, and FIDE SNP) |
| Molina | None listed | MTM (HealthChoice and FIDE SNP) |
Meridian is on hold. HFS put Meridian on an enrollment hold on August 22, 2026, under its HealthChoice Illinois contract. The sanction was over waiver members’ care plans and cuts to their service hours. While the hold lasts, Meridian does not appear as a choice, so new members cannot pick it. HFS’s letter says the hold may run into 2027. Meridian members who get waiver services may switch plans during the hold.
Humana joins HealthChoice too. Humana’s HealthChoice Illinois plan starts January 1, 2027 as well. Members locked into another plan can switch to it through March 31, 2027. HFS’s October 2026 HealthChoice subcontractor list has no Humana row yet, so ask Humana which broker books those trips.
What changes for your trips
HFS’s October 2026 subcontractor list shows Blue Cross with MTM Health and CountyCare with Modivcare. MTM Health took over Blue Cross’s Medicaid rides from Modivcare on October 1, 2026. See MTM Health in Illinois and Modivcare in Illinois.
From January 1, each rider travels through their new MDSP plan. If much of your work is CountyCare MLTSS trips through Modivcare, some of it can move to MTM Health, because the Humana and Meridian ride lines on the chart are MTM numbers. A Blue Cross rider who picks Aetna may change brokers too: Aetna’s HealthChoice plan uses Modivcare, and the chart lists no ride line for Aetna’s MDSP plan.
What to do before January 1, 2027
- List your MDSP riders. Mark every Blue Cross and CountyCare MLTSS rider, starting with standing orders for dialysis and therapy.
- Ask each facility. Ask each nursing home, supportive living facility, and dialysis unit which plan each standing-order rider is choosing. Staff can help riders call Client Enrollment Services before December 31.
- Get credentialed with MTM if you are not. Start your broker credentialing file now.
- Ask Aetna and Molina which broker will book their MDSP rides in 2027, and ask Humana which broker books its HealthChoice rides.
- Rebook January trips. Make sure each trip on or after January 1 is booked under the rider’s new plan, with that plan’s broker.
