Humana stops covering routine rides in nine of its Louisiana Medicare Advantage plan options on January 1, 2027. Its [2027 Annual Notice of Change for Humana Select Partner Plan (HMO) H1951-038](https://assets.humana.com/is/content/humana/H1951038000ANOC27pdf), sold in Jefferson, Orleans, and St. Charles parishes, changes Transportation from "$0 copayment for plan approved location up to 60 one-way trip(s) per year" to "Not Covered." The plan also ends unlimited trips for members with chronic kidney disease, end-stage renal disease, or a cancer diagnosis. Four Humana plans for people with both Medicare and Medicaid move to unlimited trips. Members can change plans until December 7, 2026.

## Which Humana plans drop or cut rides

Humana's 2027 notices for all nine make the same two cuts: the 60 one-way trips a year end, and so do the unlimited kidney and cancer trips. Together these plans are sold in 40 of Louisiana's 64 parishes, around New Orleans, Baton Rouge, Houma, Lafayette, Alexandria, and Monroe.

| Plan | Parishes |
|---|---|
| Select Partner H1951-038, LCMC Advantage H1951-051, Gold Plus H1951-059 | Jefferson, Orleans, St. Charles |
| Select Partner H1951-039 | East Baton Rouge, East Feliciana, Iberville, Livingston, St. Helena, West Baton Rouge |
| FMOL Baton Rouge H1951-053 | Ascension, East Baton Rouge, Livingston, West Baton Rouge |
| Gold Plus H1951-048, Baton Rouge North | East Feliciana, Iberville, Pointe Coupee, St. Helena |
| Gold Plus H1951-047, Bayou and River Region | Assumption, Lafourche, Plaquemines, St. Bernard, St. James, St. John the Baptist, Terrebonne |
| Gold Plus H1951-049, Lafayette | Acadia, Grant, Iberia, Lafayette, Rapides, St. Landry, St. Martin, St. Mary, Vermilion |
| Gold Plus H1951-049, Northeast | Caldwell, Catahoula, East Carroll, Franklin, Jackson, Lincoln, Madison, Morehouse, Ouachita, Richland, Tensas, Union, West Carroll |

Other versions of Gold Plus H1951-047, -048, and -049 have no rides either year. CMS's 2027 plan benefit files, released October 1, 2026, show rides in 14 of Humana's 35 Louisiana plan options sold to individuals, down from 22 of 35 in 2026. Gold Plus H1951-058, a 60-trip plan around Baton Rouge in 2026, is not offered in 2027. CMS's 2027 plan crosswalk lists it as terminated, with no plan for its members to move into.

One dual plan keeps rides but cuts them. Gold Plus SNP-DE H1951-032, renamed Humana Dual QMB Only for 2027, goes from 76 trips a year to 60 in 20 parishes around New Orleans, Baton Rouge, and Houma. Its limit per trip falls from 150 miles to 100.

## Who keeps rides or gets more

Four Humana [dual special needs plans](https://nemtguide.com/glossary/dual-special-needs-plan/) list $0 unlimited one-way trips in network for 2027, up to 100 miles a trip:

- **Gold Plus SNP-DE H1951-057 and H1951-061.** Up from 76 trips a year, in 63 parishes. The limit per trip falls from 150 miles to 100.
- **HumanaChoice SNP-DE H5216-332.** Up from 36 trips, in 20 parishes around New Orleans, Baton Rouge, and Houma.
- **HumanaChoice SNP-DE H5216-330.** Up from no rides in 2026, in 43 parishes outside the New Orleans and Baton Rouge areas.

Gold Plus H1951-013 in Bossier, Caddo, and Webster parishes keeps 60 trips a year and unlimited kidney and cancer trips. Its members call Customer Care at 800-457-4708, which directs them to the plan's [ride vendor](https://nemtguide.com/health-plans/humana-medicare-advantage/), and book at least 3 business days ahead.

Riders with full Louisiana Medicaid have another way to ride. Under LDH Informational Bulletin 15-13 (revised May 12, 2026), people with both Medicare and Medicaid get NEMT through a Healthy Louisiana plan. LDH lists [MediTrans](https://nemtguide.com/brokers/meditrans/) as the ride broker for every Healthy Louisiana plan.

## What NEMT providers should do now

1. **Check each standing rider's plan.** Ask which Humana plan they have, and compare the plan name and number with the table. Start with [dialysis](https://nemtguide.com/guides/dialysis-transportation/) and cancer riders, who lose unlimited trips.
2. **Ask your Humana ride vendor which trips end.** Ask the vendor that sends you Humana trips which standing orders stop after December 31, 2026.
3. **Tell riders before December 7.** Medicare open enrollment ends December 7, 2026. Riders can compare plans at Medicare.gov, at 1-800-MEDICARE (1-800-633-4227), or with the Louisiana Senior Health Information Program at 800-259-5300. Humana's notice adds that a rider enrolled in a Medicare Advantage plan for January 1, 2027 can still switch plans between January 1 and March 31, 2027.
4. **Point full Medicaid riders to MediTrans.** If you are not in its network yet, the [Louisiana state guide](https://nemtguide.com/states/louisiana/) explains how to join.
5. **Plan January driver hours.** Expect fewer Humana trips in these parishes from January 1, 2027. Have a [private-pay price](https://nemtguide.com/guides/private-pay-nemt/) ready for riders who stay in a plan without rides.
