# Molina Healthcare Florida CMS Plan and Medicaid Rides: MTM Health, Booking Rules, and How to Get the Trips

Molina Healthcare of Florida runs Florida's Children's Medical Services (CMS) Plan statewide from October 1, 2026, adding about 132,000 children with special health care needs to its Miami-Dade and Monroe members. MTM Health books Molina's Medicaid rides at (888) 298-4781 and booked CMS Plan rides under Sunshine. To get these trips, enroll with Florida Medicaid and join MTM's network.

Molina Healthcare of Florida's Medicaid plan serves one corner of the state, Miami-Dade and Monroe counties. On October 1, 2026, it also takes over the Children's Medical Services (CMS) Plan across Florida, so Molina members now live in every region. ## What changes on October 1, 2026

AHCA chose Molina to run the CMS Plan, which Sunshine Health runs through September 30, 2026. The plan is for children under 21 who have Medicaid and special health care needs that call for extensive ongoing care, which can mean several providers at once. AHCA's postcard to families says Molina operates the plan statewide from October 1, 2026.

Every member moves to Molina automatically, with no new application. AHCA says benefits and covered services stay the same during the move, and that existing appointments, prescriptions, and prior authorizations are honored. Its August 2026 sample letter to Medicaid families adds two dates:

### Where CMS Plan children live

AHCA counted 117,163 children in the Medicaid (Title 19) version of the plan on August 31, 2026, and another 14,894 in the KidCare (Title 21) version, which families pay a monthly premium for. Most are young: 84,417 of the Medicaid members were 1 to 13 years old.

About 29 percent of the plan's Medicaid children live in Miami-Dade. Broward, Hillsborough, Orange, and Palm Beach come next, so owners in Broward, Palm Beach, Hillsborough, and Orange County have the most CMS Plan children outside Miami-Dade.

### Rides for CMS Plan children

Under Sunshine, through September 30, 2026, MTM Health booked the plan's rides on 1-844-399-9469, from 8 a.m. to 7 p.m. Eastern on weekdays and 8 a.m. to 5 p.m. on Saturdays. Rides needed one business day of notice, and a parent, relative, caregiver, or facility staff member could book. Each child could also take up to two non-medical round trips a month, such as family visits, inside the home county. Mileage payments went through MTM at 1-888-513-0703, but never to the child's parents or guardians.

MTM is also the vendor Molina uses for its own Medicaid members, and AHCA's 2026 report lists MTM for the CMS Health Plan too. Molina tells families that from October 1 their Molina case manager will explain how to get transportation. If you carry CMS Plan children for MTM today, ask your MTM network representative in writing whether those trips stay with you after the move.

Sunshine told its providers on April 30, 2026 that their Sunshine contracts do not transfer to Molina. A van company's agreement for these trips is with MTM, not Sunshine, so MTM is the one to ask. ## Where Molina's Medicaid members live

AHCA's report for August 31, 2026 counts 55,123 Molina members in Managed Medical Assistance, the main Medicaid program, all assigned to Region I. Molina had 3,861 long-term care members, 3,329 of them getting home and community-based care rather than living in a nursing facility, and 579 in its specialty plan, almost all for serious mental illness.

No other county had more than 52 Molina Medicaid members. Miami-Dade changes the most on October 1. Add the county's 33,698 CMS Plan children to Molina's 54,196 members there and Molina reaches 87,894, more than Simply's 82,486. ### From Access2Care to MTM Health

AHCA's vendor list of July 20, 2023 named Access2Care for Molina at (888) 298-4781, and Molina's member handbook posted in June 2025 still did. MTM Health finished buying Access2Care from Global Medical Response in October 2024. Its notice to former Access2Care members says they call the same number, booked trips carry over, and MTM keeps working with the same transportation providers. ### Extra rides Molina pays for

Molina's 2026 provider manual lists two ride benefits beyond what Florida Medicaid requires:

- **Caregiver trips.** A caregiver gets four one-way trips a month to visit a member who lives in an assisted living facility, with no approval step.
- **Pathways to Prosperity trips.** Up to 10 one-way trips a month for reasons Molina defines, such as job interviews, volunteering, and support groups, with prior authorization.

## How a van company gets Molina trips

1. **Enroll with Florida Medicaid.** Florida's plans must confirm that every provider holds a current AHCA agreement. The [Florida guide](https://nemtguide.com/states/florida/) walks through enrollment and the owners' Level 2 screening.
2. **Get your county license.** Miami-Dade requires a Non-Emergency Certificate for each vehicle and a chauffeur registration for each driver, covered on the Miami-Dade page. Monroe County requires a class B certificate of public convenience and necessity from its Board of County Commissioners for any wheelchair or stretcher service. It is good for two calendar years across the county, and the renewal is filed 60 to 90 days before it expires. See Monroe County.
3. **Apply to MTM Health.** Use the Join Our Network form, then finish MTM's interview, contract, credentialing, and training. List Miami-Dade, Monroe, and every other county you can cover for CMS Plan children. MTM in Florida covers its other Florida contracts.
4. **Ask about the CMS Plan in writing.** Before you add car seats or staff for children's trips outside Miami-Dade, get MTM's answer on whether CMS Plan trips in your counties will come to you.
