# Health Net Medi-Cal Transportation: Modivcare Rides, PCS Rules, and Medical Group Trips

Health Net Medi-Cal transportation is arranged by Modivcare for members whose doctor is in Health Net's direct network or a shared-risk medical group. Health Net Community Solutions runs Medi-Cal in 10 California counties, from Los Angeles to the Sierra foothills and the Eastern Sierra. DHCS counted 1,439,001 Health Net Medi-Cal members in August 2026. Modivcare arranges rides for members of Health Net's direct network and shared-risk medical groups, while groups that carry the transportation cost book their own, so a van company has more than one way in. ## Who arranges a Health Net ride

Health Net's provider manual splits transportation by the medical group, which Health Net calls a participating physician group (PPG), that each member is assigned to. The transportation section was last updated September 10, 2025.

Three rules come with the split:

- **Everyone else books with Modivcare.** Every referral source in the Modivcare group, including PPGs, hospitals, and skilled nursing facilities, must call Modivcare. A ride arranged another way can lead to a denied claim that the PPG or hospital may have to pay.
- **Dual-risk groups must deliver.** A group with NEMT risk that does not arrange a ride in time loses it to Health Net, which books the trip and takes a deduction from the group's [capitation](https://nemtguide.com/glossary/capitation/) payment.
- **Around-the-clock rides.** Dual-risk groups and hospitals must give members 24-hour NEMT access to any pharmacy or urgent care that is open 24 hours.

Dual-risk groups give a small company a way to contract directly with the party that pays for the ride. Ask the medical groups and hospitals you already serve whether they carry NEMT risk for Health Net, and who covers their overnight trips.

Two cautions before you take a call. In Los Angeles County the Medi-Cal provider manual lists Health Net "and plan partners," so check the plan and ride line on the member's card. And the same Health Net manual serves two other plans, CalViva Health in Fresno, Kings, and Madera and Community Health Plan of Imperial Valley, whose members have their own Modivcare lines.

DHCS's managed care enrollment report, published September 11, 2026, counts Health Net's Medi-Cal members by county:

About three in four members live in Los Angeles County, where L.A. Care, the larger plan with 2,078,171 members the same month, uses Call the Car instead. See [L.A. - **Tulare.** Health Net's 130,284 members sit beside Anthem Blue Cross's 134,551, and Anthem is also a Modivcare client, so Modivcare books rides for almost every Medi-Cal plan member in the county. Kaiser Permanente has 162 there.
- **Sacramento.** Health Net is one of four plans, with Anthem Blue Cross (229,331 members), Kaiser Permanente (131,742), and Molina Healthcare (62,036). Modivcare also books rides for Anthem, the largest.
- **San Joaquin and Stanislaus.** The larger plan is Health Plan of San Joaquin, with 214,891 and 157,558 members, so apply there as well.

## How members book and how much notice you get

Members book with Modivcare at 855-253-6863 or through Health Net Member Services at 1-800-675-6110 (TTY 711). Health Net's 2026 member handbook asks them to call at least 48 hours ahead, Monday to Friday, for a routine medical ride, and at least 24 hours ahead for a car ride. Modivcare takes routine Medi-Cal reservations Monday to Friday, 7 a.m. to 7 p.m. Pacific time. Routine booking closes on weekends and six national holidays.

## The Health Net PCS form and the Care Ride Unit

Every Health Net NEMT ride needs a Physician Certification Statement (PCS) approved by Health Net's Care Ride Unit before Modivcare schedules it. The prescriber faxes Health Net form 25-183, dated February 2025, to 833-701-0051. The Care Ride Unit's phone is 833-236-9695.

- **Codes on the form.** Wheelchair van (A0130), litter or gurney van (T2005, a [stretcher van](https://nemtguide.com/glossary/stretcher-van/)), BLS (A0428), ALS (A0426), and specialty care (A0434) ambulance, and air (A0430 or A0431). A bariatric wheelchair or litter adds height and weight.
- **Air.** Needs Health Net's authorization and a letter of agreement.
- **Length.** The prescriber picks 60, 90, 180, or 365 days, and 365 only for a chronic condition. A new form is needed every 12 months.
- **Who signs.** A physician, dentist, podiatrist, physician assistant, nurse practitioner, certified nurse midwife, or mental health or substance use disorder provider.
- **No changes.** Neither Health Net nor Modivcare may modify the form once it is sent, so a rider who now needs a gurney needs a new form.

Health Net's PCS form reminds prescribers that a member who cannot walk or stand without help, including one using a walker or crutches, gets NEMT and a wheelchair van. Health Net's manual calls this ambulatory door-to-door, and Modivcare needs the same 48 hours of notice for it as for any wheelchair ride. So these riders land in your wheelchair van, with a PCS on file.

For recurring care, such as dialysis, Health Net's manual says its Medi-Cal contract has it authorize NEMT for the whole series, up to 12 months, with a standing order that guarantees assigned rides. Modivcare's California facility page describes a standing order as one request for a member who attends a program two or more days a week for at least 3 months. Rides without a PCS, the NMT group, include rideshare, sedan, taxi, paratransit such as Access, bus, and mileage reimbursement. A member who uses a folding wheelchair and moves in and out of the car alone can take a rideshare, and one who needs help moves up to a wheelchair van under NEMT.

## How your company gets Health Net trips

1. **Enroll in Medi-Cal through PAVE.** DHCS All Plan Letter 22-008 lets a plan keep a company whose enrollment is pending for only 120 days. The steps are in the [California guide](https://nemtguide.com/states/california/).
2. **Join Modivcare's California network.** Send its Become a Transportation Provider form. [Modivcare in California](https://nemtguide.com/brokers/modivcare/california/) covers the form, the insurance and driver checks, and the other plans it brings.
3. **Call the dual-risk groups and hospitals.** They need their own transportation provider for Health Net members, around the clock for pharmacy and urgent care trips.
4. **Work the facilities.** Dialysis clinics and nursing homes must book Health Net rides through Modivcare, so give them the facility line and ask them to set up standing orders.
5. **Get local permits where you drive.** In Los Angeles County that means the County ambulette license or City of Los Angeles LADOT permits, depending on where each pickup starts.
