# Blue Cross Community Health Plans Transportation: MTM Now Books BCCHP Rides in Illinois

Blue Cross Community Health Plans (BCCHP) rides are booked through MTM Health from October 1, 2026, replacing Modivcare. Members still call 877-831-3148 at least 3 days ahead, or use the MTM Link Member app. To get BCCHP trips, enroll with Illinois Medicaid in IMPACT and apply to MTM Health, BCCHP's sole ride provider from that date.

- BCCHP's rides move from Modivcare to MTM Health on October 1, 2026, and MTM carries out trips already booked for after that date.

Blue Cross Community Health Plans (BCCHP) is the Medicaid plan of Blue Cross and Blue Shield of Illinois (BCBSIL), and the largest plan in HealthChoice Illinois. On October 1, 2026, its ride broker changes from Modivcare to MTM Health. Members keep the same phone number. The company that sends you BCCHP trips does not stay the same.

## Who books BCCHP rides from October 1, 2026

BCBSIL told providers on June 26, 2026, and again on August 26, 2026, that MTM Health takes over non-emergency transportation for its Medicaid members on October 1, 2026. Its July 2026 provider newsletter calls MTM the sole provider of those rides. The HFS subcontractor list for October 2026 names MTM, Inc. as the BCCHP transportation vendor.

BCBSIL is sending eligible members a letter on how to reach MTM. As of September 30, 2026, BCCHP's member transportation page and its handbook effective January 2026 still named Modivcare and its app.

### If you ran BCCHP trips for Modivcare

Your Modivcare contract stops bringing BCCHP riders after September 30, 2026. To keep them, you need an MTM contract. Keep Modivcare too: on the October 2026 HFS list it still arranges rides for Aetna Better Health of Illinois and for [CountyCare](https://nemtguide.com/health-plans/countycare-illinois/) in Cook County. The switch makes MTM the broker for four of the six HealthChoice Illinois plans: BCCHP, Meridian, Molina, and YouthCare. On August 2026 enrollment, those four plans held 1,570,759 of the program's 2,253,817 members, about 70 percent. ## Where BCCHP members live

BCCHP serves every Illinois county, and it had 682,745 members in August 2026. Its members cluster in the Chicago suburbs. HFS's August 2026 enrollment report shows the biggest counties:

BCCHP is the largest plan in eight counties: Christian, DeKalb, DuPage, Kane, Kankakee, Kendall, McHenry, and Will. In Cook County, CountyCare is larger. In DeKalb, DuPage, Kane, Kankakee, Kendall, and Will, more than half of all HealthChoice members are in BCCHP, so a company there without an MTM contract misses most plan riders.

BCCHP also covers rides to its three Blue Door Neighborhood Centers, all in Chicago and open weekdays from 9 a.m. to 5 p.m. as of September 2026:

- Pullman, 756 E. 111th St.
- Morgan Park, 11840 S. Marshfield Ave.
- South Lawndale, 2551 W. Cermak Road

## BCCHP rules your drivers work under

The BCCHP handbook effective January 2026 tells members what to expect from the driver. Those rules were written for Modivcare trips, so ask MTM whether its BCCHP standards match before you plan a schedule around them.

Other rules decide who and what rides along:

- A parent or caregiver may ride with a child or a member with special needs. Anyone who is not the member must be approved when the ride is booked.
- A single caregiver with more than one minor child can ask Member Services to approve rides for the extra children.
- The member supplies any medical equipment or safety seat, including a wheelchair or a child's car seat.
- A member can ask not to be sent a specific transportation company again, and can file a grievance against the ride vendor.

BCCHP pays for more than doctor visits: the pharmacy right after an appointment, BCCHP events (the plans' 2024 toolkit names back-to-school and vaccination events), and the Blue Door centers. Members who live within two blocks of a bus stop can get free bus passes, requested two weeks ahead and mailed home. BCCHP also pays mileage to approved drivers, including members who drive themselves, so some riders will never need your van.

Members are told to book at least 3 days ahead. The Illinois health plans' shared transportation toolkit (updated May 31, 2024) counts that as three business days, including the day of the call but not the appointment day. It also lets BCCHP members book up to 60 days out and sends urgent trips and hospital discharges to the same reservation line.

## How to get BCCHP trips

1. **Enroll with Illinois Medicaid.** Illinois requires every transportation provider that serves plan members to enroll in IMPACT, with fingerprints and a site visit. The [Illinois state guide](https://nemtguide.com/states/illinois/) walks through it.
2. **Apply to MTM Health.** MTM's onboarding runs from application to interview, contract, credentialing, training, and a first trip. Its system will not assign trips to a provider whose credentials are out of date, and every employee completes HIPAA and fraud, waste, and abuse training each year. 3. **Ask where MTM needs vans.** MTM says it answers quickly when it needs providers in your area and keeps your file when it does not. Name your counties from the table above and say whether you run wheelchair vans, since BCCHP members give the vehicle type when they book.
4. **Set up the medical forms.** Hospitals and nursing homes fill out the HFS 2270 Physician Certification Statement, and trips from home use the HFS 2271 Certificate of Transportation Services. One form covers recurring wheelchair van or service car trips for up to 180 days. The Illinois plans' billing manual (version 36.0, June 29, 2026) sends BCCHP's forms to fax 877-272-3629.

## Ambulance trips for BCCHP members

Ambulance trips are not BCCHP's to pay. HFS moved emergency ground ambulance claims out of the HealthChoice Illinois plans on April 1, 2021, and non-emergency ground ambulance on January 1, 2022. For a non-emergency trip, the ambulance company bills HFS once Transdev, the state's prior approval vendor, approves it.

BCCHP sends members who need a non-emergency ambulance to Transdev, which its page still calls First Transit, at 877-725-0569 on weekdays from 8 a.m. to 5 p.m. The page sends every other kind of ride to the plan's ride broker, which is MTM from October 1, 2026. Transdev checks whether the member qualifies and gives a list of ambulance providers to call. Ambulance providers ask for approval in Transdev's PassPORT portal or at 866-503-9040.
