# NEMT Brokers by State in 2027: Who Manages Medicaid Rides in All 50 States and DC

It depends on the state, and in many states on the rider's health plan. As of September 2026, some states use one statewide broker, such as Georgia and New York. Some split the state into broker regions, such as Kentucky and Washington. Many let each Medicaid health plan pick its own, such as Texas and Florida. A few, such as Alabama and Maryland, use no broker.

- Check both the state and the rider's health plan, because many states let each plan use a different broker.
- Where one broker runs the whole state, such as Georgia, New Jersey, or New York, one contract reaches all Medicaid trips.
- In states with no broker, such as Alabama, the Dakotas, and Wyoming, you enroll with the state Medicaid program and bill it directly.
- Virginia, Montana, and Colorado change how rides are brokered between October 1, 2026 and January 1, 2027.
- Confirm the broker on the state's own page before you apply, because broker lists elsewhere go stale fast.

Before you apply to a broker, you need to know who actually runs Medicaid rides where you drive. In some states that is one company. In others it changes by region, by county, or by the rider's health plan. The table below shows who manages rides in every state and DC as of September 28, 2026, with a link to the official page for each.

## NEMT brokers in all 50 states and DC (September 2026)

"Fee-for-service" means members who are not in a Medicaid health plan. Where a state page gives an effective date, it is in the table.

Health plans can change ride vendors on their own schedule, so treat any plan named here as a starting point. Confirm on the plan's own transportation page before you apply.

## The five ways states run Medicaid rides

Federal rule [42 CFR 440.170(a)(4)](https://www.ecfr.gov/current/title-42/chapter-IV/subchapter-C/part-440/subpart-A/section-440.170) lets a state hire a broker to run NEMT, picked through competitive bidding. Many states do. Others pass rides to their Medicaid health plans or run the program themselves. Here is what each setup means for a transportation company.

Health plan states are where most owners get surprised. In Texas, Medicaid plans split between MTM Health and SafeRide Health, plus Modivcare until October 1, 2026, so two plans in the same county can use different brokers. ## Broker changes in 2026 and 2027

These changes are dated in the official notices. Each one moves trips from one broker to another, and some move standing orders too.

## How to find the right broker for your county and riders

1. **Open your state's row above** and read the official page. It names the broker, the regions, or the health plans.
2. **List the health plans in your county** if your state lets plans choose. Find them on the state Medicaid agency's health plan page.
3. **Check each plan's transportation page** for its ride vendor. Texas and Hawaii list every plan's vendor on one state page.
4. **Match your vehicles to the work.** A broker that mostly needs wheelchair vans in your area may not need another sedan.
5. **Call the broker's provider line** and ask whether it is adding providers where you drive, and for which vehicle types.
6. **Check Medicaid enrollment first.** Federal rule 42 CFR 438.602(b) requires states to enroll every network provider of a Medicaid health plan, and a plan can contract with you while that is pending for only up to 120 days.

A few states add a step before the broker. New York rejects new transportation enrollment applications submitted after April 15, 2024 without a letter of support from Medical Answering Services. Indiana requires IHCP enrollment before you sign with Verida. In Arizona, NEMT companies register with AHCCCS as provider type 28, which includes a site visit, an ADOT vehicle for hire permit, and AHCCCS NEMT training.

## What to do in states with no broker

Where the state runs rides, you deal with the Medicaid program itself. You enroll as a transportation provider, follow the state's billing manual, and get paid by the state or its claims contractor.

- **Alabama.** Transporters approved by Alabama Medicaid go into its NET transporters directory, and members choose from it. Transporter pay files are set up on the 1st and 16th of each month.
- **North Dakota.** Except for rides from a hospital, each ride needs prior approval on form SFN 1507 from the human service zone or tribal office, and you get a copy before the trip.
- **South Dakota.** A community transportation provider must be a government entity, a nonprofit registered with the Secretary of State, or an enrolled secure medical transportation provider.

### Which states use one statewide NEMT broker?

As of September 2026: Connecticut, Idaho, Iowa, Missouri, Rhode Island, and Wisconsin use MTM Health. Delaware, New Jersey, South Carolina, West Virginia, and Utah (for door-to-door rides) use Modivcare, and Oklahoma's SoonerRide runs through Modivcare. Georgia uses Verida, New York uses Medical Answering Services, and Vermont uses the Vermont Public Transportation Association. Montana expects Modivcare to start on October 1, 2026, and Colorado moves every trip to MediDrive on January 1, 2027.

### Which states have no NEMT broker?

Alabama, Alaska, North Dakota, South Dakota, and Wyoming run Medicaid rides through the state program, and transportation companies enroll with and bill the state. Maryland gives grants to local health departments. Pennsylvania runs its Medical Assistance Transportation Program county by county, with a broker only in Philadelphia. Colorado outside the Denver metro works with local providers until MediDrive takes every trip on January 1, 2027.

### Can one state have more than one NEMT broker?

In Nevada, MTM Health covers urban counties and SafeRide Health serves health plan members in rural counties. In Texas, each Medicaid health plan picks its own vendor, so two plans in the same county can use different brokers.

### How often do states change NEMT brokers?

States rebid broker contracts on a schedule, and health plans can switch vendors at any time. In 2026 alone, Arkansas moved Region G to Modivcare on January 2, Georgia moved its last three regions to Verida on April 1, Maine moved its midcoast region to Modivcare on July 1, and Virginia fee-for-service trips move to MTM Health on October 1. Blue Cross and Blue Shield of Texas switches to MTM Health on October 1 and Blue Cross and Blue Shield of New Mexico on November 1.

### Do I need Medicaid enrollment if I only drive for a broker?

Usually. Federal rule 42 CFR 438.602(b) requires states to screen and enroll every network provider of a Medicaid health plan, and a plan may contract with you while enrollment is pending for up to 120 days. Indiana requires IHCP enrollment before you contract with Verida. New York requires a letter of support from Medical Answering Services before you can enroll. In Montana, Modivcare takes over provider enrollment when its program starts, anticipated October 1, 2026.

### How do I find the NEMT broker for my county?

Start with the state Medicaid agency's transportation page linked in the table, which names the broker or links a county map. Then list the health plans that serve your county and check each plan's transportation page, because plans in states such as Texas, Florida, and Tennessee use different vendors. Call the broker's provider line to confirm it is adding providers where you drive.
