# MTM Health in Oregon: Trillium Tri-County Rides and the RideSource Contract (2027)

MTM Health books Oregon Health Plan rides for Trillium Community Health Plan members in Clackamas, Multnomah, and Washington counties, at 877-583-1552, and runs RideSource's brokerage operations in Lane County as Lane Transit District's contractor. To carry Trillium Tri-County trips, you apply through MTM's network page, carry $1.5 million each in general and auto liability, and take your trips through the MTM portal.

MTM Health has two jobs in Oregon, and only one of them is a network you join through MTM. It is the ride service for Trillium Community Health Plan's Tri-County plan around Portland. ## Which Oregon riders MTM books trips for

Trillium runs two [coordinated care organizations](https://nemtguide.com/glossary/coordinated-care-organization/) (CCOs) under contract with the Oregon Health Authority (OHA). OHA's CCO contact list, as of October 2026, names MTM at 877-583-1552 as the ride service for Trillium Tri-County, which covers Clackamas, Multnomah, and Washington counties. OHA's brokerage map, effective July 1, 2023, splits the same three counties the same way. Trillium's other plan, Trillium Southwest, books its rides through RideSource.

Trillium Tri-County is the smaller of the two big Portland-area plans. OHA's enrollment report counted about 66,600 Trillium Tri-County members in August 2026 and about 443,300 Health Share of Oregon members, out of about 546,000 Oregon Health Plan (OHP) members living in the three counties. So roughly one Portland-area OHP member in eight books rides with MTM. The others book with:

- **WellRide,** for Yamhill Community Care members in parts of Washington County, run by [Transdev Health Solutions](https://nemtguide.com/brokers/transdev-health-solutions/).

MTM's Trillium pages say it serves members who live in the three counties, and Trillium's provider manual (January 2026 edition) tells clinics to call the ride service for the county where the member lives, even when the visit is in another county. A Trillium member from Gresham going to a specialist in Salem is still an MTM trip, so expect some long runs out of the metro. ## MTM's role at RideSource in Lane County

Lane Transit District (LTD) owns RideSource, which books OHP rides for every member in Lane County and for Trillium members in western Linn and Douglas counties. LTD's glossary lists Medical Transportation Management, MTM's legal name, as the contractor providing RideSource brokerage operations. The provider network there belongs to RideSource, so a Eugene or Springfield company starts with the [RideSource](https://nemtguide.com/brokers/ridesource/) page, which covers LTD's contract, its review of MTM's costs, and its driver checks.

## How to join MTM's Trillium network

MTM's Trillium provider page describes three phases: an application, credentialing, and contracting. A designated contract representative works with you through each one. MTM's onboarding page adds an interview after the application and a training session before your first trip, and MTM keeps an application on file when it has no need in your area.

MTM's 2025 application asks for a state credential in some states, such as a Virginia operating authority certificate or Minnesota's STS certification. It lists none for Oregon as of October 2026, so the state steps are yours to finish before credentialing. A workable order:

1. **Set up the company.** Register with the Secretary of State, get an EIN and an NPI, and ask ODOT whether your vehicles need a Class 1R for-hire passenger permit. Form 735-9751 (January 2023 version) carries a $300 application fee, and ODOT takes questions at 503-378-6699.
2. **Apply on MTM's Join Our Network page.** Choose Oregon as your primary state, list Clackamas, Multnomah, and Washington counties, and give your vehicle types and the number of each.
3. **Buy insurance to MTM's limits** before credentialing. The next section has them.
4. **Train your drivers to the CCO rule.** OAR 410-141-3925 requires First Aid, CPR, and blood spill certification before a driver carries any member, and defensive driving and Passenger Service and Safety within three months of hire, each repeated every three years.
5. **Ask about OHA enrollment.** OAR 410-141-3565 requires providers to be enrolled with OHA, as fee-for-service or encounter-only providers, before their encounter claims are accepted. Plans enroll their providers on form OHP 3108, which lists Transportation Provider as provider type 1, so ask MTM how Trillium handles that step for you.
6. **Apply for Portland permits once you have a contract.** PBOT says new NEMT companies must first secure a contract with the local brokerage. Its resources page names Ride Connection and Kaizen Health, not MTM, so ask PBOT whether an MTM agreement qualifies. ## What MTM requires of Oregon companies

MTM's Trillium provider page sets the insurance: general and automobile liability policies of no less than $1.5 million each, with MTM listed as certificate holder and additional insured on both, plus workers' compensation that meets Oregon's requirements. Send that wording to your agent before you ask for a quote, so the certificate names MTM the way it asks. If you also need PBOT permits, compare the city's limits and buy to the higher of the two. For drivers, the Trillium page says each one must meet MTM's credentialing rules and finish all required training. Oregon's CCO rule adds a valid license for the vehicle, a criminal background check under ORS 181A.195, a clean OIG exclusion check, and a disclosure of any state drug law violation and driving history before hire. ## How Trillium Tri-County trips run

MTM books routine Trillium trips Sunday through Saturday, 8 a.m. to 5 p.m. Pacific, and urgent trips and hospital discharges 24 hours a day, every day. RideSource and the Modivcare lines for PacificSource book routine trips on weekdays only, so expect MTM trips to be booked and changed on weekends too. MTM asks members to call at least 24 hours ahead, and an urgent trip is one where the member must be seen that day.

From MTM's Trillium pages and Trillium's 2026 Rider's Guide, these rules shape your day:

- **Standing rides.** Recurring trips are booked three months at a time, or with no end date for dialysis. See [dialysis transportation](https://nemtguide.com/guides/dialysis-transportation/).
- **Returns.** A scheduled return should be picked up less than 15 minutes after the visit ends. A [will-call](https://nemtguide.com/glossary/will-call-trip/) return is dispatched when the member calls, with arrival within one hour.
- **Waiting.** Members are asked to be ready 5 minutes before pickup. Drivers wait 15 minutes past the scheduled pickup time and call the ride service before they leave.
- **Late trips.** If a late pickup or drop-off means the member cannot be seen, Trillium's guide records your company as a provider no-show, does not pay the trip, and suspends the company for disregarding the schedule. Drivers may not change a pickup time without documented permission from Trillium or the ride service.
- **Mode.** A member who says they cannot use the bus or mileage pay needs a Level of Need form from the clinic, which tells MTM which vehicle to send.
- **Rideshare.** Trillium's guide lists rideshare among its ride types for members who choose it, and those drivers wait only 5 minutes, so some trips go to rideshare drivers instead of a contracted company.
- **No requests.** MTM's Trillium member FAQ answers no when members ask whether they can request a certain transportation company, so your volume comes from MTM's assignments, not from riders asking for you.

Trillium's provider manual says ride services verify appointments for non-urgent same-day trips and for a share of other rides. MTM pays nothing for no-shows, sets each company's rates one at a time, and sends every trip through the MTM Health portal. ## How Trillium checks the trips you bill

On September 17, 2026, the Oregon Department of Justice announced guilty pleas by two Portland NEMT owners who also drove for their companies and had billed for rides for people who had died. Trillium referred both cases to the department's Medicaid Fraud Control Unit. The pleas came on September 10 and 15, 2026, and each owner got 36 months of supervised probation and a ban on running any business that receives Medicaid funding.

OHA's 2026 CCO contract makes every plan check that the rides it pays for were approved and actually given, including a yearly analysis that matches ride claims against the claims for the visits they served, and audit a share of daily rides for pickup and drop-off times. MTM's standard agreement, in the January 1, 2023 version Pennsylvania posts, has each vehicle send its location, trip events, and rider signatures in real time, so every trip you bill MTM should match a real ride with that GPS trail and signature. ## What changes in 2027 and 2028

Two dates matter for MTM's Portland-area work:

1. **January 2027.** OHA's notice, updated August 7, 2026, says all Healthier Oregon members move from CCOs to open card that month. Healthier Oregon gives full OHP benefits regardless of immigration status. Those Trillium members will then book with Tri-County MedLink, so an open card agreement keeps them on your schedule.
2. **December 31, 2028.** OHA's 2026 CCO contract runs from January 1, 2020, renews a year at a time, and can last through that date before a successor contract. OHA is planning the next round of CCO contracts. Ask MTM how long your agreement runs and what happens to it if Trillium's contract changes.
