# MTM Health in Ohio: Four Medicaid Plans and How to Join (2027)

MTM Health books Ohio Medicaid rides for four health plans: AmeriHealth Caritas Ohio, Anthem Blue Cross and Blue Shield, Humana Healthy Horizons in Ohio, and Molina, including Molina's MyCare members. Each plan has its own MTM ride line. To get those trips, hold a PUCO certificate, license and enroll any wheelchair vans with the state, then apply on MTM's Join Our Network page.

MTM Health, based in Lake St. Louis, Missouri, books Medicaid rides for four of Ohio's seven health plans. ## Which Ohio plans use MTM

These plans name MTM Health on their own member pages and manuals, Molina's as of spring 2026 and the others as of September 2026. MTM's own app page also lists Molina Medicaid in Ohio, as of September 2026.

MTM takes one application per state, so a single Ohio application puts you in line for riders from all four plans. The other plans use other brokers: CareSource uses [Provide A Ride](https://nemtguide.com/brokers/provide-a-ride/), and Buckeye uses [SafeRide Health](https://nemtguide.com/brokers/saferide-health/ohio/). UnitedHealthcare's 2026 provider manual names Provide A Ride for stretcher and ambulance rides, and Provide A Ride's ride request page lists UnitedHealthcare Ohio. ### How MTM got its Ohio plans

When MTM joined the Ohio Association of Health Plans on January 5, 2023, it said it would start rides for two of the association's health plans within the first two months of that year. Anthem and Molina came to MTM later, through Access2Care, the broker MTM bought from Global Medical Response in a purchase it announced as final on October 8, 2024.

Molina told its members that their ride app would change from Access2Care's to MTM Link on February 5, 2026, with the same phone number, the same benefits, and booked rides carried over. Anthem's October 2025 provider manual names MTM Health, while its provider benefits page still names Access2Care at the same 800-282-9720 line as of September 2026. MTM's notice to former Access2Care members says it keeps working with the same transportation providers. ## Which Ohio riders turn into van trips

Many riders never reach a van. AmeriHealth Caritas's January 2026 provider manual says MTM arranges the most cost-effective mode available. Anthem's member handbook lets members who walk ask for rideshare, a bus pass, or mileage for a driver they bring. Molina offers Lyft or Uber, taxis or vans, bus passes, and mileage, and AmeriHealth Caritas offers bus day passes and pays family mileage on trip logs sent within 60 days. The work left for a van company is riders who need a hand, riders in wheelchairs, and long or repeated medical trips.

Those repeat trips are the steady part, because each plan lists rides with no yearly cap:

Anthem's 90-day bookings for dialysis, chemotherapy, and radiation work like [standing orders](https://nemtguide.com/guides/nemt-standing-orders/), and its handbook tells members that a [will-call](https://nemtguide.com/glossary/will-call-trip/) return should arrive within 30 minutes of the call. Two child rules reach your drivers. AmeriHealth Caritas and Molina members bring their own car seats, and AmeriHealth Caritas members must install them. ## Ohio's trip rules MTM has to meet

The state's contract with its health plans sets the floor under every health plan trip MTM books in Ohio. The Ohio Department of Medicaid's contract, revised July 2026, makes each plan hold its ride vendor to these rules:

| Special needs | Door-to-door help, attendant support, and rider-specific timing passed on to the vendor |

The same contract creates extra work. For children in OhioRISE, Ohio's program for complex behavioral health needs, each plan must arrange rides for parents, caregivers, and siblings in the home when the child's treatment needs them, and the AmeriHealth Caritas and Anthem manuals add that this holds even when the child is not riding. Each plan must also add transportation for members under 21 that has no yearly limit, and specialized rides for members with cognitive or behavioral challenges. Each plan also files a winter weather plan with ODM that sets the snow emergency levels that cancel trips and says how members are told. ## How to join MTM in Ohio

1. **Get your Ohio registrations.** A company that carries riders for pay needs a PUCO certificate of public convenience and necessity under section 4921.03. Wheelchair vans also need an [ambulette](https://nemtguide.com/glossary/ambulette/) license from the State Board of EMFTS, $100 a year, plus a $50 permit fee and a $50 inspection fee for each van every year under rule 4766-3-03. 2. **Enroll wheelchair vans with Ohio Medicaid.** MTM's application, as of September 2026, shows an Ohio requirement: wheelchair providers need a valid Ohio EMS license and an ODM ID, and applications without both are voided. You get the ODM ID by enrolling in ODM's Provider Network Management portal, where the 2026 application fee is $750.
3. **Get an NPI.** MTM's form asks for your EIN, NPI, and Medicaid ID. See [how to get an NPI number](https://nemtguide.com/guides/how-to-get-an-npi-number-for-nemt/).
4. **Apply on MTM's Join Our Network page.** List the Ohio counties you serve on a consistent basis, your vehicles by type, your dispatch software, and your insurance. A network representative contacts you if your area needs providers, and then come an interview, a contract, credentialing, and training.
5. **Insure to Ohio's floors.** PUCO requires $1.5 million in public liability for vehicles seating 15 or fewer, driver included, and $5 million for 16 or more, under rule 4901:2-13-03. An ambulette license adds $500,000 per occurrence and in the aggregate, plus $100,000 per person, $300,000 per accident, and $50,000 in property coverage on each vehicle, under section 4766.06. 6. **Renew on time every year.** The ambulette license, each van's permit, and each van's inspection renew yearly under rule 4766-3-03, and MTM's system will not assign trips to a provider whose credentials have lapsed. ### Hamilton County trips through MTM Transit

MTM Transit is an affiliate that MTM Health's leadership team created in 2009 to run transit services. On July 16, 2026, Hamilton County's commissioners approved a $10,521,347.11 agreement with MTM Transit LLC for Medicaid NET rides for adults and group rides for minors, next to a second agreement with Valley Transport LLC. That is county work, booked by Hamilton County Job and Family Services rather than a health plan, and MTM's application lists Hamilton County's Medicaid NET program among its choices. ## What MTM pays in Ohio

Ohio rule 5160-15-28, effective August 1, 2026, takes rides arranged by a health plan off the state's fee schedule and leaves their payment to the plan's contract with ODM. The rate you get for an MTM trip is the one in your MTM agreement. For members outside the plans, ODM pays wheelchair vans up to these amounts, unchanged since January 1, 2024:

## What to do first

1. **Enroll your wheelchair vans with ODM** before you apply, since MTM voids Ohio wheelchair applications without an ODM ID.
2. **Buy insurance to PUCO's $1.5 million floor**, and add the ambulette limits if you run wheelchair vans.
3. **Apply to MTM** and name every county you serve on a consistent basis.
4. **Ask your network representative two questions:** which of the four plans your contract covers, and how Anthem's 90-day dialysis bookings get assigned.
5. **Apply to Provide A Ride and SafeRide too**, so CareSource and Buckeye riders in your area can ride with you.
6. **Call your county job and family services office** about short trips, and in Hamilton County file the HCJFS interested vendor form.
