# MTM Health in Arizona: AHCCCS Plan Rides Through MTM, MTBA, and Veyo (2027)

Most AHCCCS plan members in Arizona get their rides through MTM Health. Arizona Complete Health, UnitedHealthcare Community Plan, Health Choice, and Molina Complete Care name MTM, its MTBA unit, or MTM/Veyo as their ride vendor, and Banner sends clinics to MTM's ride portal. To drive these trips, register with AHCCCS as provider type 28, then apply to MTM's network and finish any plan credentialing.

- Five of Arizona's six AHCCCS Complete Care plans point members or clinics to MTM, so MTM's network is the way to most plan members' rides.
- In the five northern counties, both acute care plans name MTM, and all three acute plans in southern Arizona point members or clinics to it.

Arizona has no statewide Medicaid ride broker. Each AHCCCS health plan buys rides its own way, and most of them now point to MTM Health, which owns two companies that held Arizona plan contracts before it: Veyo and Medical Transportation Brokerage of Arizona (MTBA). This page sorts out which Arizona plans run through MTM, under which name, and what that means for a company that wants their trips.

## Which Arizona plans send rides to MTM

AHCCCS has six AHCCCS Complete Care (ACC) plans for acute care. As of 2026, five of them point members or clinics to MTM, or a company MTM owns, in their own manuals, member pages, or provider sites. AHCCCS's count for September 2026 shows how many members each one has:

Together the five have 1,069,747 members, about three in four of the 1,404,311 people in AHCCCS acute care plans. Each plan describes its link to MTM in its own words:

- **Arizona Complete Health.** It made MTBA its only NEMT vendor on October 1, 2021, dropping Veyo, before MTM owned either. Its prior authorization pages now say transportation is handled by "MTM (formerly MTBA)." It is the only ACC plan in all 15 counties, and its own rules are on the [Arizona Complete Health page](https://nemtguide.com/brokers/arizona-complete-health/).
- **UnitedHealthcare Community Plan.** Its care provider manual for 2027, issued in September 2026, covers ACC members, ALTCS members who are elderly or physically disabled, and members with developmental disabilities. It sends all of them to MTM at 1-888-889-0358, asks for three calendar days of notice, lists MTM's scheduling hours as weekdays from 6 a.m. to 7 p.m. Arizona time, and lets clinics book in MTM's portal.
- **Health Choice.** Blue Cross Blue Shield of Arizona Health Choice names MTM as its NEMT broker in section 3.13 of its provider manual, revised March 13, 2026, with member lines 602-386-2447 and 800-322-8670 (option 2, then option 1). Rides to behavioral health visits may be set up by the member's behavioral health home instead.
- **Molina Complete Care.** Its member page, as of May 2026, says Molina partners with "MTM/Veyo." Members book weekdays from 8 a.m. to 6 p.m., three business days ahead, for trips in the current or next month. Urgent trips such as discharges, dialysis, cancer treatment, and surgery are handled the same day within a one-hour window, and members under 18 ride with an adult 21 or older.
- **Banner University Family Care.** Members book through Banner's Customer Care Center at least 72 hours ahead. Banner's provider site sends clinics to MTM Link to request, change, and cancel their patients' rides and see where the driver is, with a guide dated August 2024. Banner's 2027 member handbook and July 2026 provider manual name no vendor, so ask Banner which one carries its members. Banner also credentials NEMT vendors and transportation companies itself, which its manual says can take up to 90 days.

Mercy Care, the sixth ACC plan, with 285,876 members, does not name its broker. An AHCCCS letter of September 2025 refers only to "the NEMT broker" behind its ride line, and Mercy Care contracts transportation companies through its own Network Management team; see [Mercy Care transportation](https://nemtguide.com/health-plans/mercy-care-arizona/). Fee-for-service members, including the 92,629 people in the American Indian Health Program in September 2026, sit outside the plans altogether. AHCCCS pays registered companies directly for their trips and requires prior authorization for trips over 100 miles.

## Where MTM's plans cover

The plan map in AHCCCS's list revised June 30, 2026 shows why MTM matters most outside the Phoenix area:

- **North: Apache, Coconino, Mohave, Navajo, and Yavapai counties.** Both acute care plans there, Arizona Complete Health and Health Choice, name MTM. UnitedHealthcare, also an MTM plan, covers the area's ALTCS members who are elderly or physically disabled.
- **South: Cochise, Graham, Greenlee, La Paz, Pima, Santa Cruz, and Yuma counties, plus Pinal ZIP codes 85542, 85192, and 85550.** The acute care plans are Arizona Complete Health, Banner, and, in Pima County, UnitedHealthcare, and all three point members or clinics to MTM.
- **Central: Maricopa, Gila, and the rest of Pinal County.** All six plans compete here, so a Phoenix company that wants Mercy Care members as well needs a second contract.

AHCCCS pays its own fee-for-service trips that start outside the Phoenix and Tucson metro areas at separate rural rates, billed with the TN modifier, but MTM pays what your agreement says. ## What MTM already runs in Arizona

MTM is not only a broker here. When it bought MTBA, the purchase closing July 1, 2023, MTBA had been running since 1987 and carried about one million trips a year with more than 600 vehicles and 700 drivers. It also ran taxis in Phoenix and Tucson through AAA Cab Services, and MTBA's leader stayed on as general manager.

MTM bought Veyo in August 2022, and Veyo recruits MTM's VeyoRide drivers: independent contractors who drive their own cars. MTM's April 1, 2026 update lists Arizona among the eight states where VeyoRide was serving Medicaid and Medicare members, and says MTM can send last-minute discharges, will-call returns, and surges in demand to those drivers with real-time incentives. AHCCCS Policy 310-BB lets transportation network companies serve members only through a broker.

That gives MTM its own vehicles, taxis in Phoenix and Tucson, and independent drivers alongside the companies it subcontracts. ## How to get MTM trips in Arizona

1. **Register with AHCCCS first.** Every NEMT company, plan work or not, registers as provider type 28, a high risk type. That means the owner's state NEMT course, a site visit, fingerprints, and the $750 enrollment fee for calendar 2026. The [state guide](https://nemtguide.com/states/arizona/) walks through the portal.
2. **Apply to MTM.** Use the Join Our Network form on MTM's service providers page. MTM says that where it needs providers you hear back quickly, and where it does not, it keeps your information and calls when an opening comes up.
3. **Expect a plan to credential you too.** Arizona Complete Health's manual says any subcontracted transportation provider must be credentialed with both the plan and its broker, and Banner credentials transportation companies through the Arizona Association of Health Plans application. 4. **Bill MTM under your agreement.** Arizona Complete Health's manual has companies under a direct contract with its broker bill trips and loaded miles as that agreement sets out, with the AHCCCS Daily Trip Report and full pickup and drop-off addresses on every claim. ## The rules the plans hold MTM to

Each plan's AHCCCS contract sets the trip standard, and the plan audits its ride vendor against it, which in practice means auditing your trips. Arizona Complete Health's contract, as amended October 1, 2026, has members arrive no more than an hour early and wait no more than an hour after treatment, and it sets a target of 95 percent of pickups and drop-offs on time each quarter. Booking notice changed on October 1, 2026. Policy 310-BB now lets a plan require no more than three calendar days of notice, makes it try to fill shorter requests, and requires exceptions such as dialysis, chemotherapy, and hospital discharges. The plans still word it their own way: UnitedHealthcare asks for three calendar days, Arizona Complete Health for three days, Banner for 72 hours, and Molina's page in May 2026 for three business days. ## Contracts and changes to watch

The ACC contracts run through September 30, 2027, and Arizona's SB 1741 of 2025, Section 22, requires AHCCCS to extend every ACC and ACC-RBHA contract one more year, through September 30, 2028. A plan that changes its transportation vendor must submit the change to AHCCCS as a material change to business operations under that contract. Vendors have changed before: Arizona Complete Health swapped Veyo for MTBA in 2021. Plans change too. Care1st's northern members moved to Arizona Complete Health on October 1, 2024, and AHCCCS's September 2026 count shows Care1st with no members, so treat any old Care1st ride line as gone.

## What to do first

1. **Start AHCCCS registration** as provider type 28, since no plan or MTM contract can pay you without it.
2. **Pick your counties.** North and south, the acute care plans all point to MTM. In Maricopa, Gila, and most of Pinal, plan to approach Mercy Care as well.
3. **Apply to MTM** and name the vehicle types and counties you can cover.
4. **Prepare the plan application** that Arizona Complete Health and Banner use, with your vehicle maintenance schedule and car seat records.
5. **Set dispatch for the one-hour rules:** no arrival more than an hour early, and a ride home within an hour of the member's call.
6. **Mark September 30, 2028,** the end of the extended plan contracts, when the plan lineup, and with it the ride vendors, can change.
