# Modivcare in Oregon: PacificSource Rides in Central Oregon, the Gorge, and Marion-Polk (2027)

Modivcare books Oregon Health Plan rides for PacificSource Community Solutions members in three regions: Central Oregon (Crook, Deschutes, Jefferson, and north Klamath), the Columbia Gorge (Hood River and Wasco), and Marion and Polk counties. To get those trips, you join Modivcare's network through its provider form. Open card members in the same counties book with other brokerages, so plan on more than one contract.

- Modivcare books OHP rides for PacificSource Community Solutions members in Central Oregon, the Columbia Gorge, and Marion and Polk counties.
- PacificSource had about 239,000 members in those three regions in August 2026, more than 147,000 of them in Marion and Polk counties.
- Open card members in the same counties ride with Cascades East Transit, GOBHI, Marion-Polk MedLink, or TransLink, so one contract does not cover a whole county.
- Drivers on PacificSource trips may help a rider up one or two steps and into the lobby, but enter a home only for a hospital discharge or a stretcher ride.

In Oregon's Medicaid program, Modivcare works for one plan. PacificSource Community Solutions, the [coordinated care organization](https://nemtguide.com/glossary/coordinated-care-organization/) (CCO) for Central Oregon, the Columbia Gorge, and the Salem area, sends its members' Oregon Health Plan (OHP) rides to Modivcare. Companies in Bend, Redmond, Madras, Hood River, The Dalles, and Salem get PacificSource work by contracting with Modivcare, while other ride services handle everyone else. ## The three PacificSource regions Modivcare covers

PacificSource's provider manual, dated September 2026, names Modivcare Solutions as its NEMT brokerage for all three of its regions. Each region has its own reservation line. The member counts come from OHA's enrollment report for August 2026.

That is about 239,400 PacificSource members whose rides go through Modivcare. PacificSource is the main plan in each region. In Marion and Polk counties, about 147,400 of the roughly 162,700 OHP members belonged to it in August 2026, and in Hood River and Wasco about 17,700 of roughly 19,000. Marion-Polk holds about six in ten of the three regions' members, so most PacificSource trips start in the Salem area.

The plan's Rider's Guide (effective January 1, 2026, updated February 2026) sets the hours: Monday through Friday, 9 a.m. to 5 p.m., with urgent and hospital discharge trips taken 24 hours a day, 7 days a week. The call center may close on New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas. Each region also has a separate Ride Assist line: 855-397-3620 for Central Oregon, 855-397-3618 for the Gorge, and 844-544-1398 for Marion-Polk.

In Central Oregon, the Central Oregon Intergovernmental Council's ride center brokered PacificSource rides until April 1, 2020. That day LogistiCare, Modivcare's former name, took them over, and the council's Cascades East Transit took over the open card rides. OHA's brokerage map, effective July 1, 2023, still shows LogistiCare for PacificSource in all three regions. OHA's CCO contact page, as of October 2026, lists RideSource for the Central Oregon region, but RideSource is Lane County's ride service, and PacificSource's own manual, Rider's Guide, and ride page all give Modivcare's lines.

## Riders in the same counties who book elsewhere

A PacificSource contract does not cover every Medicaid rider in a county. OHA's 2026 open card handbook sends open card members, who are not in a CCO, to these brokerages:

- **Klamath.** [TransLink](https://nemtguide.com/brokers/translink-rvtd/), at 888-518-8160, which also books for Cascade Health Alliance, Klamath County's other CCO.

Yamhill Community Care members who live in parts of Polk County book with WellRide, run by [Transdev Health Solutions](https://nemtguide.com/brokers/transdev-health-solutions/), at 844-256-5720. Lane County is no longer PacificSource territory. OHA's 2026 CCO changes page says PacificSource handled Lane County members through January 31, 2026, and Trillium took them from February 1. Lane County riders book with [RideSource](https://nemtguide.com/brokers/ridesource/). To fill a van with Medicaid riders across a whole county, plan on two or three agreements. The Rider's Guide also covers members with both Medicaid and Medicare, including PacificSource Dual Care members, who can ride to appointments covered by either plan. Modivcare's Oregon facility page lists separate ride lines for Centene's Allwell and Health Net plans (877-718-4201) and for HCSC group retirees (866-824-1566). It still lists a HealthSpring line too, but HealthSpring moved its Medicare Advantage rides from Modivcare to MTM Health on March 1, 2026.

## How to get Modivcare's PacificSource trips

PacificSource's provider manual, written for its clinics, says the brokerage is responsible for providing the rides and that clinics follow the guidelines and processes the brokerage sets. Your contract, your rate, and your trips all come from Modivcare, not from PacificSource. Modivcare's Oregon web page is written for clinics and members, so ask its network team for Oregon's insurance limits and rates before you buy a policy. A workable order:

1. **Check ODOT first.** Ask ODOT's Commerce and Compliance Division at 503-378-6699 whether your vehicles need a Class 1R for-hire passenger permit. Form 735-9751 (January 2023 version) carries a $300 application fee.
2. **Send Modivcare's provider form.** The Become a Transportation Provider form asks for your NPI, tax ID, services, vehicle count, hours, and primary service state, and Oregon is on its list. Give the county your vans are based in.
3. **Buy insurance to Modivcare's list.** Its join page asks for general liability, sexual abuse and molestation coverage, auto liability, and workers' compensation, with limits that vary by state. The [Modivcare guide](https://nemtguide.com/brokers/modivcare/) has the full list.
4. **Train drivers to the CCO rule.** OAR 410-141-3925 requires First Aid, CPR, and blood spill certification before a driver carries a PacificSource member, and defensive driving and Passenger Service and Safety within three months of hire.
5. **Ask about OHA enrollment.** OAR 410-141-3565 requires providers to be enrolled with OHA before their encounter claims are accepted, and plans enroll their providers on form OHP 3108. Ask Modivcare how PacificSource handles that step.
6. **Pass credentialing.** Modivcare checks drivers, vehicles, and insurance, and its join page lists field visits for quality assurance. Prepare with the [credentialing checklist](https://nemtguide.com/templates/nemt-credentialing-checklist/).

Under OHA's 2026 CCO contract, PacificSource itself must document each driver's criminal history, driving history, sex offender status, and drug testing, verify each subcontractor's workers' compensation, general liability, and auto insurance, and audit a share of daily rides for pickup and drop-off times. ## What PacificSource tells riders to expect from your drivers

Riders get the 2026 Rider's Guide from their plan, so it is the standard they will hold your drivers to on Modivcare trips.

### At the door

- **Arriving.** The driver calls, knocks, or comes into the lobby. Phones and tablets are used hands-free.
- **Help.** If the rider asks, the driver helps up or down one or two steps, between the door and the vehicle, and into the building lobby. A rider who needs more brings an attendant.
- **Limits.** Drivers do not enter the home or room except for a hospital discharge or a stretcher ride. They do not help the rider dress, move a rider between bed and wheelchair or wheelchair and van, take money, sell anything, or make their own stops.

### Riders and equipment

- **Seat belts.** Everyone wears one, and riders in wheelchairs use both the lap and shoulder belt. A driver may refuse a rider who will not buckle up. Riders with a seat belt exemption card show it before every ride, and riders who need a longer belt say so when they book, so keep extenders on board.
- **Attendants and children.** Up to two adult attendants ride at no cost. Children 12 and under ride with an adult, and the family brings and installs the car seat and takes it along, because the return may be a different driver.
- **Large chairs.** Riders tell Modivcare at booking if their wheelchair is wider than 30 inches, longer than 48 inches, or over 600 pounds in use, and if they use a scooter or oxygen. ### Timing and problems

- **Pickup windows.** The guide gives a 9:00 to 9:30 window as its example and asks riders to be ready at the start. Riders never have to board before the scheduled time, and drivers wait 15 minutes, then tell the ride provider. The drop-off, return, and late rules are the statewide CCO ones in the [Oregon guide](https://nemtguide.com/states/oregon/).
- **No-shows.** A rider who is not ready and did not cancel is a no-show. Riders with many no-shows or same-day cancellations can be limited to a certain transportation company, an attendant, or the bus.
- **Crashes.** A crash with an injury or death is reported by the plan, the ride provider, and the driver within two working days of learning of it, with the driver, the passenger, the place, the time, and any hospital care.
- **Bad weather.** In heat, cold, flooding, snow, or ice, rides to dialysis, chemotherapy, and other critical care come first, and the ride provider keeps a backup plan for vehicles more than 15 minutes late. See [dialysis transportation](https://nemtguide.com/guides/dialysis-transportation/).

## What changes in 2027 and 2028

Two dates move PacificSource riders:

1. **January 2027.** OHA's notice, updated August 7, 2026, says all Healthier Oregon members move from CCOs to open card that month. In Modivcare's regions, those riders switch to Cascades East Transit, GOBHI, Marion-Polk MedLink, or TransLink. An open card agreement in your county keeps them on your schedule.
2. **December 31, 2028.** OHA's 2026 CCO contract runs from January 1, 2020 and can be renewed a year at a time through that date, followed by a successor contract. OHA is planning the next round. PacificSource's exit from Lane County in 2026 shows that a plan's map can change, so ask Modivcare how long your agreement runs.
